DECLARATION
of 24 July 2013
laying down models of forms and languages of the Member States of the European Union used in cross-border cases to provide financial assistance to victims of crime
According to § 49 (a) to (e) of Act No. 45 / 2013 Coll., on Victims of Crime and on the amendment of certain laws (Act on Victims of Crime):
§ 1
This decree provides
(a) a model of the application form for cash assistance in cross-border cases;
(b) a model form for the transmission of requests for cash assistance in cross-border cases;
(c) a model form for the transmission of decisions on requests for assistance in cross-border cases;
(d) other than the official languages in which requests for assistance and annexes thereto are made, additional information and a list of all documents referred to, requests for additional information and information pursuant to Article 36 (2) of the Law on victims of crime;
(e) the official languages of the other Member States of the European Union in which the Ministry of Justice will receive requests for assistance in cash, their annexes and additional information and, where appropriate, a list of all documents referred to.
§ 2
(1) The model of the application form for cash assistance in cross-border cases is set out in Annex 1 to this Decree.
(2) The model form for the transmission of requests for cash assistance in cross-border cases is set out in Annex 2 to this Decree.
(3) The model of the form for the transmission of decisions on the application for monetary assistance in cross-border cases is set out in Annex 3 to this Decree.
§ 3
Other than the official languages in which requests for cash assistance and annexes thereto are made, additional information and a list of all documents referred to, requests for additional information and information pursuant to Article 36 (2) of the Law on victims of crime are given in Annex 4 to this Order.
§ 4
In cross-border cases, the Ministry of Justice shall accept requests for assistance, annexes and additional information, or a list of all documents referred to, in Slovak and English.
§ 5
This Decree shall take effect on 1 August 2013.
