DECLARATION
of 2 October 2020
on training in the basic strains of doctors
According to § 37 (1) (n), (p) and (q) of Act No. 95 / 2004 Coll., on the conditions for obtaining and recognising professional competence and specialised competence for the medical profession of physician, dental practitioner and pharmacist, as amended by Act No. 67 / 2017 Coll.:
This decree lays down requirements for:
(a) the breakdown and scope of training in the basic strains of doctors (hereinafter referred to as the "basic strain");
(b) the content of the basic tribe education;
(c) technical and material equipment and personnel security which are a condition for obtaining accreditation for the implementation of specialisation training in the basic tribe;
(d) the type of workplace on which compulsory vocational experience is carried out in basic school training;
(e) the length of compulsory professional experience in basic school education;
(f) theoretical knowledge and practical skills which are the subject of post-school examinations in the baseline tribe;
(g) the scope and content of the study obligations which must be completed before applying for the test after completion of the basic group training.
The requirements of Section 1 are set out for the baseline strain
(a) anaesthesiological in Annex 1 thereto,
(b) dermatoenerological in Annex 2 to this Decree,
(c) gynecological / obstetrics in Annex 3 to this Decree;
(d) hygienic-epidemiological in Annex 4 thereto;
(e) surgical in Annex 5 thereto,
(f) internal in Annex 6 to this Order,
(g) cardiothoracic surgery in Annex 7 thereto,
(h) maxi-facialsurgical in Annex 8 to this Decree;
(i) neurosurgical in Annex 9 to this Decree;
(j) neurological in Annex 10 to this Decree;
(k) ophthalmic in Annex 11 to this Decree,
(l) orthopaedic in Annex 12 thereto,
(m) Otorinolaryngological in Annex 13 to this Decree,
(n) pathology in Annex 14 to this Decree,
(o) paediatric in Annex 15 to this Decree,
(p) psychiatric in Annex 16 to this Decree,
(q) radiological in Annex 17 to this Decree,
(r) urological in Annex 18 to this Decree; and
(s) general medical practice in Annex 19 thereto.
Transitional provisions
(1) The completion of the professional experience which the doctors have completed since 1 July 2017 in the field of specialisation training with the health service provider, which, on 30 June 2017, was accredited by an accredited establishment accredited under the training programmes in force before the date of entry into force of Decree No. 221 / 2018 Coll., and provided such training after 30 June 2017 and at the latest before the date of entry into force of that decree, has submitted an application for accreditation under Decree No. 221 / 2018 Coll., is deemed to be compulsory professional practice which was completed in an establishment accredited under Decree No. 221 / 2018 Coll., but at the latest until the date of the legal authority of the decision on the application for accreditation.
(2) The completion of the professional experience which the doctors, as from 1 July 2017 in the field of specialisation in paediatric education, have completed with the health service provider, who, on 30 June 2017, had submitted an application for accreditation pursuant to Decree No. 221 / 2018 Coll., before the date of entry into force of Decree No. 221 / 2018 Coll., is considered to be compulsory professional experience for the field of paediatric education which was completed in an establishment accredited under Decree No. 221 / 2018 Coll., but at the latest until the date of entry into force of the legal authority of the application for accreditation.
(3) A doctor who started and failed to complete the training in the base tribe before the date of entry into force of this decree may complete the training in accordance with Decree No. 221 / 2018 Coll. or according to this decree.
(4) The accreditation granted for the provision of education in the base tribe by the health service provider under Decree No. 221 / 2018 Coll., is considered to be the accreditation granted for the respective base strain under that decree.
(5) The accreditation procedure initiated before the date of entry into force of this Order shall be completed in accordance with this Order.
Decree No 221 / 2018 Coll., on education in the basic strains of doctors, is repealed.
Efficacy
This Decision shall enter into force on 15 October 2020.

Requirements for technical and material equipment and personnel security for workplaces which are a condition for obtaining accreditation to carry out specialist medical training
For Part I. (a) 1.
Anesthesiology and Intensive Medicine
(a) technical equipment
The health service provider meets the requirements for the technical and material equipment of the emergency bed intensive care workplace in 2nd degree - higher intensive care or emergency bed intensive care in 3rd degree - resuscitation care according to Annex No. 4 Requirements for the technical and material equipment of the health care facility for Decree No. 92 / 2012 Coll., on the requirements for the minimum technical and material equipment of health care facilities and home care contact centres.
(b) Personnel security
The health service provider meets the requirements for minimum staff security of the workplace of the acute intensive care of the second degree - higher intensive care or acute intensive care of the third degree - resuscitation care according to Annex No. 3 Requirements for the personnel security of the bed care for Decree No. 99 / 2012 Coll., on the requirements for minimum personal care of the health services.
(c) type of workplace
The health service provider is authorised to provide bed care in the field of anesthesiology and intensive medicine or to provide acute intensive bed care in the 2nd degree - higher intensive care or acute intensive care in the 3rd degree - resuscitation care.
The health service provider shall provide anesthesiological care in the field of surgery and in at least one of the following fields:
1. orthopaedic surgery and trauma of the musculoskeletal system,
2. otorinolaryngology and head and neck surgery,
3rd urology,
4. gyno and obstetrics
Health care in the above mentioned fields is provided in the form of bed care for which it is entitled. This condition will also be met by workplaces with a common bed fund for these unions.
The health service provider provides acute bed intensive and resuscitation care on beds of a multidisciplinary nature or a multidisciplinary intensive care unit with the possibility of ventilation, where the management is attended by a doctor anaesthesiologist with specialised competence in the field of anesthesiology and intensive medicine.
Acute intake to intensive / resuscitation beds: at least 100 patients per calendar year or on average 100 patients per year over the last 3 calendar years.
Anesthesia administered: at least 1 500 per calendar year or on average 1 500 per year for the last 3 calendar years.
for Part I. (a) 2.
Surgery
(a) technical equipment
The health service provider complies with the requirements for technical and physical equipment of the health care facility of the relevant field according to Decree No. 92 / 2012 Coll.
(b) Personnel security
The health service provider complies with the requirements for minimum staff security of the health care facility of the relevant field according to Decree No. 99 / 2012 Coll.
Health care shall be provided under a continuous working regime of at least:
1. a doctor with specialised competence in surgery;
2. by a doctor with a specialised competence in the field of surgery or by a doctor assigned to specialist training in the field of surgery with a minimum of 3 months of experience since inclusion;
3. a doctor with specialist competence in anesthesiology and intensive medicine,
4. a physician with specialised competence in radiology and imaging methods,
5. a physician with specialised competence in the field of internal medicine.
The availability of a doctor with specialised competence in gyno and obstetrics is ensured. The availability of a doctor with specialist competence in gynaecology and obstetrics is understood to mean the availability of advice and assistance by telephone or electronically, if requested, physical presence depending on the urgency of the performance and the clinical condition of the patient.
The provider providing compulsory professional experience may also provide the availability of a doctor with specialist competence in the field of gyno and obstetrics through a health service provider accredited to provide education in the basic gynecological and obstetrics tribe or own specialised training in the field of gyno and obstetrics with which he enters into a contract.
(c) type of workplace
The health service provider is authorised to provide bed care in the field of surgery.
The facility has at least 2 operating rooms.
Components of the health service provider for which professional experience is carried out:
1. bed separation of anesthesiology and intensive medicine, or intensive care unit, or multidisciplinary unit, with possibility of at least short-term artificial pulmonary ventilation,
2nd Radiodiagnostic Division,
3. internal department,
4. endoscopy workplace,
5. has services in the field: hematology and transfusion medicine, clinical biochemistry, medical microbiology and pathology, gyno and obstetrics.
Number of patients hospitalised per calendar year: at least 1 000 or on average 1 000 per year for the last 3 calendar years.
Number of outpatient outpatient patients per calendar year: at least 10 000 or an average of 10 000 per year for the last 3 calendar years.
List of performance types and their number per calendar year:
1. planned operations: 700 or on average 700 per year for the last 3 calendar years;
2. acute operations: at least 200 or on average 200 per year for the last 3 calendar years,
of which acute or planned laparoscopic operations: at least 100 or 100 per year for the last 3 calendar years,
3. trauma treatment: at least 500 of which at least 30 osteosynthesis or on average 500 per year for the last 3 calendar years and 30 osteosynthesis per year.
for Part I. (a) 3.
Internal medicine
(a) technical equipment
The health service provider complies with the requirements for technical and physical equipment of the health care facility of the relevant field according to Decree No. 92 / 2012 Coll.
(b) Personnel security
The health service provider complies with the requirements for minimum staff security of the health care facility of the relevant field according to Decree No. 99 / 2012 Coll.
At least 2 doctors who meet the conditions for internal medical trainers with a total minimum time of 1.5 for a given health service provider.
(c) type of workplace
The health service provider shall be authorised to provide inpatient care in the field of internal medicine.
The health service provider shall meet the following care requirements:
1. income internal ambulance,
2. acute bed care with the condition of providing acute bed care intensive,
3. provision of basic bed care for patients in the field of internal medicine,
4. outpatient care in the field of internal medicine,
5. diagnostic complement in own or contract equipment with 24-hour availability.
6. patient intake in the whole spectrum of internal diseases: at least 1000 income per health service provider per year or on average 1000 income per year over the last 3 calendar years,
7. outpatient care: at least 2000 patients per calendar year in ambulances providing professional care in the spectrum of internal diseases or on average 2000 patients per year for the last 3 calendar years.
for Part I. (a) 4.
Anesthesiology and Intensive Medicine - 24 months
(a) technical equipment
The health service provider complies with the requirements for the technical and material equipment of the acute intensive care centre of the 3rd degree - resuscitation care Annex No. 4 Requirements for technical and material equipment of the health care facility of the bed care facility for Decree No. 92 / 2012 Coll., on the requirements for minimum technical and material equipment of the health care facility and contact home care centres.
(b) Personnel security
The health service provider meets the requirements for minimum staff security of the workplace of acute intensive care in the 3rd degree - resuscitation care according to Annex No. 3 Requirements for staff security of bed care for Decree No. 99 / 2012 Coll., on requirements for minimum staff security of health services.
(c) type of workplace
The health service provider is authorised to provide bed care in the field of anesthesiology and intensive medicine or acute intensive care in the 3rd degree - resuscitation care.
The health service provider shall provide anaesthesiological care at its own workplace for at least 2 of the following subjects, the provision of which in berth form is authorised:
1. surgery,
2. gynecology and obstetrics,
3. orthopaedic surgery and trauma of the musculoskeletal system,
4. otorinolaryngology and head and neck surgery,
5th urology.
The workplace provides acute bed care intensive in beds of a multidisciplinary character, or in a multidisciplinary intensive care unit under the guidance of a doctor anaesthesiologist.
Acute intake to intensive / resuscitation beds: at least 100 patients per calendar year or on average 100 per year for the last 3 calendar years.
Anesthesia administered: at least 3 000 per calendar year or on average 3 000 per year for the last 3 calendar years.

Urgent medicine
(a) technical equipment
The health service provider meets the requirements for technical and material equipment of urgent income according to Decree No. 92 / 2012 Coll.
The health rescue service provider complies with the requirements for technical and material equipment of medical emergency services according to the Decree No. 92 / 2012 Coll.
(b) Personnel security
The health service provider meets the requirements for minimum personnel security of urgent income according to Decree No. 99 / 2012 Coll.
The health rescue service provider complies with the requirements for the personnel transport of patients urgent care according to Decree No. 99 / 2012 Coll.
(c) type of workplace
Provider of medical services providing medical services in the field of emergency medicine or provider of medical rescue services.
Educational activities with entrusted organisations

Textroom for theoretical instruction.
Access to the operating rooms and the berth CPR station of the branch workplace
(b) Personnel security
Lectors with specialised competence in anaesthesiology and intensive medicine and experience for at least 5 years in the field, possibly with specialised competence or specific professional competence or special specialised competence in the subject.

1. Endorsement of compulsory professional experience in the field confirmed by the trainer with the relevant specialised competence, or by the trainer according to the provisions of § 38a (2) (c) of Law No. 95 / 2004 Coll. in accredited equipment within the specified scope.
2. The completion of compulsory training activities in Part I (b).
3. Confirmation of mandatory performance by the instructor.
