DECLARATION
of 16 March 2011
amending Decree No. 296 / 2004 Coll., implementing the Act on the Registration of Citizens and amending Decree No. 177 / 2000 Coll., implementing the Act on the Registration of Citizens, the Act on Civil Documents and the Act on Travel Documents, as amended, as amended
According to § 25 (a) and (b) of Act No. 133 / 2000 Coll., on the registration of residents and birth numbers and on the amendment of certain laws (the Act on the registration of residents), as amended by Act No. 53 / 2004 Coll.:
Decree No. 296 / 2004 Coll., implementing the Act on the Registration of Residents and amending Decree No. 177 / 2000 Coll., implementing the Act on the Registration of Residents, the Act on Civil Documents and the Act on Travel Documents, as amended, as amended, as amended by Decree No. 661 / 2004 Coll. and Decree No. 299 / 2006 Coll., is hereby amended as follows:
1. In the title of the decree, the words "and amending Decree No. 177 / 2000 Coll., implementing the Act on the Registration of Citizens, the Act on Civil Documents and the Act on Travel Documents, as amended, 'are deleted.
2. In Paragraph 1, the sentence "The procedure referred to in the second sentence shall not apply where the assembly or requested birth numbers referred to in paragraph 2 are provided by remote access via an electronic communications network providing back communication with the point of departure."
3. in Article 1 (3) (a), "telecommunications networks" shall be replaced by "electronic communications networks."
4. in Article 2 (2) (e), including footnote 7,
"(e) in the case of adoption, the name and / or surname of the adopter (s) (7), the surname and birth number of the acquirer (s);
7) Act No. 94 / 1963 Coll., on Family, as amended. '
5. Part Two, including the title, is deleted.
6. Annex 3 shall read as follows:
"Annex 3 to Decree No. 296 / 2004 Coll.
APPLICATION FOR PROVISION OF DATA FROM THE INFORMATION SYSTEM
"
Transitional provision
The form of the request for data from the information system issued in accordance with the model of this form provided for in Decree No. 296 / 2004 Coll., as effective until the date of entry into force of this Order, may be used until 31 December 2015 at the latest.
Efficacy
This decree shall take effect on the day of its publication.
