GOVERNMENT REGULATION
of 15 December 2004
on the model of the service card of an inspector of the Office for the Protection of Personal Data and the model of the service card of an authorised staff member of the Office for the Protection of Personal Data, carrying out a control activity
The Government orders the implementation of § 38 paragraph 5 (a) of Act No. 101 / 2000 Coll., on the Protection of Personal Data and on the Amendment of Certain Acts, as amended by Act No. 439 / 2004 Coll.:
The model and the description of the service card of the personal data protection officer proving himself in the performance of the tasks and powers of the personal data protection officer shall be as set out in Annex 1 to this Regulation.
The model and the description of the service card of an authorised staff member of the Office for the Protection of Personal Data, carrying out an inspection activity, which shall be demonstrated in the performance of the tasks and the exercise of the powers of the authorised staff of the Office for the Protection of Personal Data, carrying out a control activity, are set out in Annex 2 to this Regulation.
This Regulation shall enter into force on the day of its publication.

The reverse side of the service card
The inspector's service pass shall be in the form of a laminated card of size 74 x 105 mm with rounded corners. The dimensions of the paper core are 68 x 99 mm.
Paper core color - grey yellow background with blue textile fibers.
The face side of the service card has a large state emblem on the top left. In addition to the great state emblem, the inscription "PERSONAL DATA PROTECTION AUTHORITY 'and the inscription" INSPECTOR INSPECTOR INSPECTOR INSPECTION' appear below. The face of the service card shall also bear the name and surname of the holder, the number of the licence and the date of issue. On the bottom left is a photograph of the holder with dimensions 35 x 45 mm. The requirements for the technical execution of the photograph correspond to the technical requirements for photographs for the issue of the ID 1).
The reverse side of the upper part of the service card shall contain information on the holder's legal authorisation to carry out the inspection activity, the lower part shall include a space for a round stamp of the Office of Personal Data Protection and the President's signature by hand, as well as information on the use of the service card.

The controlling service card is in the form of a laminated card with dimensions 74 x 105 mm with rounded corners. The dimensions of the paper core are 68 x 99 mm.
Paper core color - grey yellow background with blue textile fibers.
The face side of the service card has a large state emblem on the top left. In addition to the large national emblem, the inscription "PERSONAL DATA PROTECTION AUTHORITY 'and the inscription" TRAINING OF AN EMPLOYMENT CERTIFICATE' appear below. The face of the service card shall also bear the name and surname of the holder, the number of the licence and the date of issue. On the bottom left is a photograph of the holder with dimensions 35 x 45 mm.
