DECLARATION
of 9 May 2018
laying down the formalities and models of certain educational documents for accredited qualification courses, certified courses and specialisation training
According to Section 90 (2) (c) of Act No. 96 / 2004 Coll., on the conditions for obtaining and recognising the eligibility for the pursuit of non-medical medical professions and for carrying out activities related to the provision of health care and amending certain related laws (Act on non-medical medical medical professions), as amended by Act No. 201 / 2017 Coll.:
Subject matter
This decree regulates the requirements and models
(a) a certificate of professional competence for the profession of health;
(b) a certificate of specific competence obtained;
(c) a diploma on specialisation;
(d) a certificate of professional competence for the pursuit of the profession of another professional worker;
(hereinafter referred to as education documents).
Forms of educational documents
(1) The general requirements of the educational documents are:
(a) the designation of the Ministry or accredited establishment issuing the training document, namely the name, registered office and identification number of the person;
(b) the particulars of the person to whom the education document is issued, namely the academic title, the name and surname, the date of birth, the place of birth;
(c) the number of the educational document; and
(d) the place, date of issue of the educational document and the signature of the person acting for the Ministry or persons acting for the accredited establishment.
(2) The specific requirements of certificates of professional competence acquired for the pursuit of the health profession are:
(a) an indication of the period of completion of the accredited qualification course;
(b) the designation of the accredited qualification course;
(c) the number of the accreditation decisions,
(d) indication of the date of completion of the final examination;
(e) the designation of the health profession;
(f) the stamp (1) of the accredited establishment; and
(g) the signature of the expert guarantor of the course and the chair of the examination committee.
(3) The specific requirements of the certificate of specific competence obtained are:
(a) the name of the certified course;
(b) the number of the accreditation decisions,
(c) indication of the number of hours of the training programme;
(d) an indication of the period of completion of the training programme;
(e) an indication of the activities for which the graduate of the certified course has acquired specific competence, including the definition of the activities which are the provision of health care;
(f) the stamp (1) of the accredited establishment; and
(g) signature of the expert guarantor of the course.
(4) The special requirements of the diploma on specialisation are:
(a) indication of the date of completion of the test;
(b) a designation of the field of specialised competence;
(c) the designation of the specialist;
(d) the stamp (2) of the Ministry; and
(e) the signature of the Chairman of the Technical Attestation Committee and the signature of the person acting for the designated organisation through which the Ministry issues the diploma of specialisation.
(5) The specific requirements of certificates of professional competence acquired for the pursuit of the profession of another professional worker are:
(a) an indication of the period of completion of the accredited qualification course;
(b) the designation of the accredited qualification course;
(c) the number of the accreditation decisions,
(d) indication of the date of completion of the final examination;
(e) an indication of the profession of another professional;
(f) the stamp (1) of the accredited establishment; and
(g) the signature of the expert guarantor of the course and the chair of the examination committee.
Additional requirements for educational documents
(1) Education documents shall be drawn up on a sheet 210 x 280 mm with a tolerance of 3 mm and 100 grams of paper.
(2) The evidence of education is bipartisan, with a grey underprint with a motif of a small state emblem and a motif of a lipid branch.
Models of educational documents
(1) The model certificate of professional competence for the exercise of the profession of health is set out in Annex 1 to this Decree.
(2) The model certificate of specific competence obtained is set out in Annex 2 to this Decree.
(3) The model of the diploma on specialisation is set out in Annex 3 to this Decree.
(4) A model certificate of professional competence for another professional is given in Annex 4 to this Decree.
Efficacy
This decree shall take effect on the 15th day following its publication.
