DECLARATION
Ministry of Environment
of 13 January 2000
determining the extent and manner of processing of major accident reports and final reports on the occurrence and consequences of a major accident
According to Article 15 (4) of Act No 353 / 1999 Coll., on the prevention of major accidents caused by selected hazardous chemicals and chemical preparations and amending Act No 425 / 1990 Coll., on District Offices, the modification of their responsibilities and on certain other measures related thereto, as amended, (Act on the Prevention of Major Accidents) ("the Act '):
The written report on the occurrence of a major accident shall be drawn up as set out in Annex 1.
The final written report on the occurrence and consequences of the major accident shall be prepared to the extent specified in Annex 2.
The report on the occurrence of the accident and the final written report on the occurrence and consequences of the major accident shall be presented in one original copy and in electronic form at the same time.
This Decree shall take effect on 29 January 2000.
