DECLARATION
of 6 March 2013
on the extent of information provided on chemical mixtures having certain dangerous properties and detergents
According to § 37 (2) of Act No. 350 / 2011 Coll., on Chemicals and Chemical Mixtures (Chemical Act), the Ministry of Health provides for the implementation of § 22 (6) of the Chemical Act:
Subject matter
(1) This Decree sets out the scope of information provided in electronic form to the Ministry of Health on:
(a) chemical mixtures having dangerous physicochemical properties or dangerous health-affecting properties ("dangerous mixtures"), first placed on the market of the European Union in the Czech Republic by importers and downstream users,
(b) dangerous mixtures from another Member State of the European Union first marketed in the Czech Republic by suppliers.
(2) This decree also sets out the scope of information provided in electronic form to the Ministry of Health on detergents:
(a) first placed on the market of the European Union in the territory of the Czech Republic by manufacturers,
(b) from another Member State of the European Union first marketed in the Czech Republic by distributors.
Scope of information
(1) The scope of the information provided on hazardous mixtures referred to in Article 1 (1) (a) is laid down in Part One of Annex 1 to this Decree.
(2) The scope of the information provided on hazardous mixtures referred to in Article 1 (1) (b) is laid down in Part Two of Annex 1 to this Decree.
(3) The scope of the information provided on detergents under Article 1 (2) (a) is laid down in Part One of Annex 2 to this Decree.
(4) The scope of the information provided on detergents under Article 1 (2) (b) is set out in Part Two of Annex 2 to this Decree.
Efficacy
This Decree shall take effect on 15 March 2013.

1.2. Relevant intended uses of dangerous mixtures
1.3. Information on the legal or business person responsible for placing a dangerous mixture on the market (position of the person in the distribution chain, name or name of person, address of registered office, name of contact person, contact telephone and e-mail address)
2. IDENTIFICATION OF DANGEROUS
2.1 Classification of dangerous mixtures (according to the chemical law or Regulation (EC) No 1272 / 2008 of the European Parliament and of the Council of 16 December 2008 on classification, labelling and packaging of substances and mixtures, amending and repealing Directives 67 / 548 / EEC and 1999 / 45 / EC and amending Regulation (EC) No 1907 / 2006, as amended)
3. INFORMATION ON THE SAFETY JUSTICE
3.1 Hazardous substances contained in the mixture and substances for which the European Union exposure limit for the working environment is established, and substances which are persistent, bioaccumulative and toxic (PBT) and highly persistent and highly toxic (vPvB), including their precise representation in the mixture, including their classification
