DECLARATION
of 25 January 2016
amending Decree No. 490 / 2000 of the Ministry of Health Coll., on the scope of knowledge and other conditions for obtaining professional competence in certain fields of public health protection, as amended by Decree No. 472 / 2006 Coll.
According to Article 108 (1) of the Act No. 258 / 2000 Coll., on the Protection of Public Health and on the Amendment of Certain Related Acts, as amended by Act No. 274 / 2001 Coll., Act No. 320 / 2002 Coll., Act No. 274 / 2003 Coll., Act No. 392 / 2005 Coll., Act No. 222 / 2006 Coll., Act No. 151 / 2011 Coll., Act No. 223 / 2013 Coll. and Act No. 267 / 2015 Coll., for the implementation of § § 19 (3), § 59 (2), § 60 (1) and (3) and § 83e (2) Act:
Decree No. 490 / 2000 Coll., on the extent of knowledge and other conditions for obtaining professional competence in certain fields of public health protection, as amended by Decree No. 472 / 2006 Coll., is amended as follows:
1. In the introductory sentence of the Decree, the words "Paragraph 19 (4) 'are replaced by" Paragraph 19 (3)' and the words "and § 83e (2) 'are inserted after the words" Paragraph 60 (1) and (3)'.
2. in Article 1 (c):
"(c) the composition of test committees, the conditions for carrying out the examination in the fields referred to in (a), the models of proficiency certificates and the model of certificate of authorisation for the assessment of health risks.";
3. in Article 3 (1) (a):
"(a) the name and, where applicable, the name, surname, academic title, date of birth and address of the place of permanent residence or residence, if the person does not have permanent residence in the Czech Republic,"
4. the following Section 8 is inserted after Section 7:
The model of the certificate of professional competence referred to in § 58 (1) to (3) of the Act is set out in Annex 5 to this Order. '
5. the following Article 11b is inserted after Article 11a:
The model of the authorisation certificate provided for in Section 83e (1) of the Act is set out in Annex 6 to this Decree. '
6. The following Annex 5 is inserted after Annex 4:
"Annex No 5 to Decree No. 490 / 2000 Coll.
A. Model of the certificate of professional competence under Section 58 (1) of the Public Health Protection Act
B. Model of certificate of professional competence according to Section 58 (2) of the Public Health Protection Act
C. Model certificate of professional competence according to § 58 (3) of the Act on Public Health Protection
7. the following Annex 6 is inserted after Annex 5:
"Annex 6 to Decree No 490 / 2000 Coll.
Model of the certificate of authorisation under Section 83e (1) of the Public Health Protection Act
"
Efficacy
This Decision shall enter into force on 15 February 2016.
