DECLARATION
of 22 December 2014
on certain documents relating to education
The Ministry of Education, Youth and Sports provides, pursuant to § 28 paragraphs 7 and 8 of Act No. 561 / 2004 Coll., on pre-school, primary, secondary, higher vocational and other education (Education Act), as amended by Act No. 179 / 2006 Coll.:
Content and form of forms of educational documents
(1) The forms of certificates, forms of education, diplomas of discharge and the certificate of a professional specialist shall be accompanied by a grey underprint with the motif of a small national character and of lipid leaves on each side of the form.
(2) Forms of certificates, teaching papers and diplomas for the evaluation of pupils and students in primary education, secondary education, secondary education with a certificate, secondary education with a graduate examination and higher vocational education shall be further provided, except as provided for in paragraph 1,
(a) watermark with the motifs of the lipos;
(b) a description of the series of forms and at least a six-digit number of the form; and
(c) the place of presentation of the graphic code, which shall appear on the relevant forms in the abbreviation "QR code"; in the graphic code there is always the name, surname and date of birth of the rated person, the department identifier of the school, the designation of the field of education, the school year, the classification in compulsory subjects and the overall evaluation.
(3) The forms referred to in paragraph 2 shall be drawn up on a sheet or, where appropriate, a double sheet 210 x 280 mm with a tolerance of 3 mm. The other forms referred to in paragraph 1 shall be drawn up on a sheet or, where appropriate, a double sheet 210 x 297 mm with a tolerance of 3 mm.
(4) The location of the school's department identifier shall be indicated on the relevant forms by the abbreviation "IZO." The place of listing of the department identifier of the legal person carrying out the school's activities shall be given by the abbreviation "RED IZO 'in the graduation certificate. The place for indicating the level of qualification achieved under the European Framework of Qualifications for Lifelong Learning shall be indicated by the abbreviation" EQF'.
(5) The certificate shows the field of education with the secondary education with the certificate and the secondary education with the graduate examination (5) and the additional field of education with the certificate.
Models of forms of educational documents
(1
(a) primary schools in Annex 1 to this Decree;
(b) secondary schools and conservatories in Annex 2 thereto.
(2) Models of the forms of the training documents are given in the event of the achievement of:
(a) primary education in primary school, secondary school or in courses for basic education and in the case of basic education in primary school special in Annex 3 thereto;
(b) secondary and secondary education with a certificate in Annex 4 thereto;
(c) secondary education with a graduate examination in Annex 5 thereto;
(d) higher vocational training in the Conservatory in Annex 6 to this Decree;
(e) higher vocational education in Annex 7 thereto.
(3) Models of the certificate forms are given in the case of:
(a) the professional certificate in Annex 7a thereto,
(b) the Certificate of State Language Examination in Annex 8 to this Decree;
(c) primary art schools in Annex 9 to this Decree;
(d) a certificate of the state examination of graphic disciplines in Annex 10 thereto.
Models of forms for primary school evaluation verbally
(1) The primary school of assessment shall use the model forms set out in Annex 1 or Annex 1a for the purpose of assessing the results of the education of the pupil at the end of the semester.
(2) The primary school of assessment shall use the model forms set out in Annex 3 or Annex 3a for the evaluation of the results of the education in the event of the achievement of the degree of education.
Content and form of extracts from the certificate
The extract of the certificate shall contain the particulars resulting from the model form of the certificate set out in the Annexes to this Order. The extract of the certificate shall always be provided in the name, name, surname and signature of the person issuing the extract, where applicable.
Copy and copy of the certificate, certificate and diploma of discharge
(1) A copy of the certificate, the certificate of education, the diploma of discharge and the certificate of professional specialist (hereinafter referred to as "copy") shall be issued by the school at the request of the person to whom the original was issued or to its legal representative. The copy shall be issued as a copy of the original submitted and shall be accompanied by the endorsement "This copy conforms literally to the original" together with the stamp of the legal person performing the school's activities (hereinafter referred to as "the stamp"), the name, surname and signature of the person issuing the copy, and the date of issue of the copy. Where the original consists of several separate sheets, these particulars shall be indicated on each sheet. The issue of the copy shall be indicated in the relevant school documentation.
(2) The same copy of the certificate, the certificate of education, the diploma of discharge and the certificate of professional specialist (hereinafter referred to as the copy) shall be issued by the school at the request of the person to whom the original was issued or to its legal representative. The same copy shall be drawn up on the appropriate form in force at the time of issue of the original or, if this form is not available, on another appropriate form. The issue of a copy shall be recorded in the relevant school documentation.
(3) Where the signatures of the persons who have signed the original cannot be affixed, the original shall appear on the copy of the name and surname of the persons who have "v. r. '. Where the stamp used on the original cannot be affixed, the abbreviation" L. S. "shall appear on the copy instead of that imprint. The endorsement" This copy conforms to the original' shall be affixed to the copy. The following particulars shall appear on the document: the name, surname and signature of the principal of the school and the name of the legal person issuing the copy, the stamp and the date of the copy. Where a copy is made out on several separate sheets, each copy shall be accompanied by a document and particulars.
(4) Where a school which has issued an original does not already exist, a copy shall be issued in accordance with the procedure laid down in paragraphs 2 and 3 by the school which imposes its documentation, or by another school providing education in the same or similar field of education, provided that the applicant submits to him copies of the documents on the basis of which the copy may be issued, drawn up in accordance with the Charter and File Service Act (1) or certified under the Verification Act (2).
(5) If the school documentation to which the application for a copy relates has been destroyed, the school shall issue a certificate to the applicant.
(6) Where a copy is issued because of a change in the name, surname or birth number, the applicant's name and surname and the birth number shall be indicated on the copy as at the date of issue of the copy. The application for the issue of a copy shall be accompanied by an original of the certificate, a certificate of discharge or a certificate of professional expertise and by evidence of change of name, surname and birth number. The change of name or surname shall be evidenced by a decision of the competent administrative authority authorising the change of name and surname (3) or, where appropriate, an officially certified copy thereof. The change in the birth number shall be demonstrated in accordance with another legislation4).
In the case of forms of certificates of state examinations from graphic disciplines, the tasks of the school referred to in paragraphs 1 to 6 shall be performed by the Ministry of Education, Youth and Sports set up and the legal person responsible for the examinations.
Repeal
The following shall be deleted:
1. Decree No. 223 / 2005 Coll., on certain documents on education.

6. Decree No. 86 / 2013 Coll., amending Decree No. 223 / 2005 Coll., on certain documents on education, as amended.
Transitional provision
Until 30 September 2017, the forms provided for in Decree No. 223 / 2005 Coll., as effective before the date of entry into force of this Order, may be used.
Efficacy
This Decree shall take effect on 15 January 2015.
