DECLARATION
Ministry of Industry and Trade
of 29 December 1998
on the form and content of a safety data sheet for a dangerous chemical and preparation
The Ministry of Industry and Trade provides pursuant to Section 14 (3) of Act No. 157 / 1998 Coll., on Chemicals and Chemicals and on the amendment of certain other laws (hereinafter referred to as "the Act"):
§ 1
Safety data sheet 1) to a hazardous chemical (hereinafter referred to as "substance ') and the preparation shall be processed in the Czech language in written or electronic form.
§ 2
(1) The safety data sheet shall state:
(a) identification of the substance or preparation and identification of their manufacturer or importer;
(b) information on the composition of the substance or preparation;
(c) information on the hazards of the substance or preparation;
(d) first aid guidelines;
(e) arrangements for emergency action in the event of a fire caused by a substance or a preparation or in the vicinity of a substance or preparation;
(f) measures in the event of accidental leakage of a substance or preparation;
(g) guidance on the treatment of the substance or preparation and the storage of the substance or preparation;
(h) how to control the exposure of persons to the substance or preparation and the protection of persons;
(i) information on the physical and chemical properties of the substance or preparation;
(j) information on the stability and reactivity of the substance or preparation;
(k) information on the toxicological properties of the substance or preparation, indicating whether the data have been obtained by animal testing;
(l) environmental information on the substance or preparation;
(m) information on the disposal of the substance or preparation;
(n) information for the transport of the substance or preparation;
(o) information on legislation relating to the substance or preparation,
(p) further information.
(2) The model of the safety data sheet form and the instructions for drawing it up are set out in the Annex to this Regulation.
(3) New serious information2) concerning the substance or preparation is provided on a new safety data sheet marked as a revision.
§ 3
This decree shall take effect on the day of its publication.

GUIDELINES FOR CONSTRUCTION OF THE SAFETY LIST
1. Identification of the substance or preparation and identification of the manufacturer or importer
To be indicated
1.1 for the substance or preparation
1.1.1. The expression for the identification of a substance or preparation identical to that given on the labelling of the packaging of a substance or preparation,
Other names, e.g. group name, trivial name, trade name, 3)
1.1.3 other means of identification, where appropriate,
1.2 for the manufacturer or importer
1.2.1. Name, residence, place of business, identification number and telephone number of the natural person or business name, registered office, identification number and telephone number of the legal person,
1.2.2 an emergency telephone number providing emergency information (e.g. accidents, threats to human life and health) related to the substance or preparation.
2. Information on the composition of the substance or preparation
To be indicated
2.1. Chemical names of ingredients or additives
2.1.1 the chemical names of substances referred to in § 2 (8) (a) to (n) of the Act, including their concentrations or concentration ranges where substances are present in concentrations equal to or greater than those referred to in § 3 (3) (a) to (c) of the Act,
2.1.2. Chemical names of substances for which maximum permissible concentrations in the working air are determined on the basis of a specific regulation, (4) in a descriptive manner only the characteristics of the chemical structure of the substance or preparation to ensure the safe handling of the substance or preparation, where the composition of the substance or preparation is subject to commercial secrecy,
2.2 Hazard warning symbols and R- phrases. 5)
3. Data on the hazards of the substance or preparation
To be indicated
3.1 the most serious risks posed by the substance or preparation, in particular serious risks to human health and the environment,
3.2. The most serious adverse effects on human health and the expected adverse effects arising from the usual or incorrect use of the substance or preparation.
4. Instructions for first aid
Specify by mode of exposure of the individual by substance or preparation (inhalation, skin contact, eye contact, ingestion)
4.1 First aid instructions, in particular whether immediate medical advice should be sought or the presence of a doctor recommended,
4.2. Symptoms and effects of exposure to humans by substance or preparation,
4.3. Actions to be taken at the scene of an accident in the event of an accident;
4.4. Indication whether the long-term effects of this exposure may be expected after exposure to the person by the substance or preparation,
4.5 an indication of the need for specific devices at workplaces to provide specific immediate treatment after exposure to humans with a substance or preparation, for certain substances and preparations.
5. Measures for emergency action in the event of a fire caused by a substance or a preparation, or in the vicinity of a substance or preparation
To be indicated
5.1 suitable fire extinguishers,
5.2. Fire extinguishers which cannot be used for safety reasons,
5.3. Specific hazards caused by the substance or preparation, burning products, resulting from gases,
5.4 Special protective equipment for firefighters.
6. Measures in case of accidental leakage of a substance or preparation
To be indicated
6.1 Recommended safety measures for the protection of human health (e.g. removal of ignition sources, drawing up and, where appropriate, putting up instructions on adequate ventilation or protection of the respiratory organs, carrying out dust checks, avoiding contact of the substance or preparation with skin or eyes),
6.2 recommended environmental safety measures (e.g. keeping the substance or preparation out of the sewerage, outside surface and groundwater, outside the soil, indicating the need to alert the environment),
6.3 recommended cleaning methods (e.g. use of sorption material, reduction of gas or smoke content by water or dilution).
7. Instructions for the treatment of the substance and preparation and storage of the substance or preparation
To be indicated
7.1. For the treatment of a substance or preparation
7.1.1 precautions for the safe handling of the substance or preparation, including data on technical measures (e.g. local and overall ventilation, precautions against aerosol and dust formation and fire),
7.1.2 other specific requirements or rules relating to the substance or preparation (e.g. prohibited or recommended warehouse procedures and equipment),
7.2 for storage
7.2.1 conditions for safe storage [e.g. indication of the specific construction of storage areas or containers (including storage walls), indication of ventilation, indication of the material with which the substance or preparation must not come into contact, indication of the temperature range and humidity of storage areas, indication of the light conditions of storage spaces, indication of inert gas in storage spaces, indication of specific electrical equipment and indication of the avoidance of static electricity in storage places],
7.2.2 the maximum authorised quantity of the substance or preparation for the storage conditions,
7.2.3 specific requirements (e.g. the type of material used on the packaging or container) for the substance or preparation.
8. Method of controlling exposure of persons with a substance or product and protection of persons
To be indicated
8.1 technical measures to minimise exposure of persons (e.g. information on the design of workplaces),
8.2 control parameters (e.g. limit values of the substance or preparation or biological standards),
8.3 recommended procedures for monitoring human exposure,
8.4 types of personal protective equipment
8.4.1 for breathing protection (e.g. breathing apparatus, filter protective mask) in the case of hazardous gases, vapours or dust,
8.4.2 for eye protection (e.g. safety glasses, goggles, face protection),
8.4.3 for hand protection (e.g. gloves, hand and skin protection measures) when handling a substance or preparation,
8.4.4 for the protection of the skin (e.g. apron, shoes and protective clothing, hygiene measures), if other parts of the body need to be protected in addition to the hands.
9. Information on the physical and chemical properties of the substance or preparation
Indicate
9.1. Condition at 20 ° C (solid or preparation, liquid, gas) and colouring of the substance or preparation,
9.2. The odour or smell of the substance or preparation;
9.3 the pH of the substance or preparation in the supplied form or in an aqueous solution; in the case of an aqueous solution, indicate its concentration;
9.4 melting temperature or melting temperature range of the substance or preparation,
9.5 boiling temperature or boiling range of substance or preparation,
9.6 point of ignition of the substance or preparation,
9.7. Flammability of the substance or preparation,
9.8. Self-ignition of the substance or preparation,
9.9. Explosives of the substance or preparation (upper and lower limits),
9.10 oxidative properties of the substance or preparation,
9.11 vapour tension of a substance or preparation,
9.12 density of substance or preparation,
9.13 solubility of the substance or preparation in water and fats, including the partition coefficient n-octanol / water,
9.14 additional data (e.g. vapour density, mixing ability, evaporation rate, conductivity, viscosity).
10. Information on the stability and reactivity of the substance or preparation
To be indicated
10.1 conditions (e.g. temperature, pressure, light, impact) which may cause a dangerous reaction of the substance or preparation and which need to be avoided and a description thereof,
10.2 substances and materials (e.g. water, air, acids, bases, oxidising agents or other specific substances) which may cause a dangerous reaction and with which the substance or preparation must not come into contact and describe them,
10.3 hazardous substances arising from degradation of the substance or preparation in dangerous quantities, including data on:
10.3.1 the need for a stabiliser in a substance or preparation,
10.3.2 possibility of a dangerous exothermic reaction,
10.3.3 the importance of changing physical properties (e.g. appearance, colour, morphology) for the stability and safety of the substance or preparation,
10.3.4 dangerous breakdown products of the substance and preparation, arising from contact with water,
10.3.5 possibility of degradation of the substance or preparation into unstable products.
11. Information on the toxicological properties of the substance or preparation
To be indicated
11.1 a full description of all (immediate, long and chronic) toxicological effects of the substance or preparation on human health arising from both experience and scientific experience which may arise from short-term or long-term exposure to the substance or preparation (e.g. sensitisation, carcinogenicity, mutagenicity, reproductive toxicity, narcotic effects),
11.2. Information on the different ways of exposure of a person to a substance or preparation (e.g. inhalation, ingestion, skin contact, eye contact) and the symptoms of its effects in relation to the physical, chemical and toxicological properties of the substance or preparation,
11.3 for certain preparations, references to the specific effects of their constituents on human health,
11.4 whether data have been obtained by animal testing.
12. Environmental information on the substance or preparation
To be indicated
12.1 the effects, behaviour and breakdown of the substance or preparation or their degradation products in the environment in relation to the properties of the substance or preparation (mobility, degradability, cumulation, short-term and long-term effects on ecology),
12.2 other adverse effects of the substance or preparation on the environment (e.g. ozone depletion, photochemical ozone formation, global warming, sewage effects in the treatment plants),
12.3. Unresolved areas associated with the assessment of the environmental effects of a substance or a preparation, only if substances (separately or as part of a preparation) are present and classified as hazardous to the environment.
13. Information on the possibility of disposal of the substance or preparation and packaging
To be indicated
13.1 a description of the disposal of surpluses or wastes of a substance or preparation resulting from the intended use of a substance or preparation where their disposal poses a risk to human health or the environment and information on the safe handling of such surpluses or waste;
13.2 appropriate methods of disposal of the substance or preparation and contaminated packaging (e.g. incineration, recycling, landfill),
13.3 references to waste legislation. 6), 7)
14. Information for the transport of a substance or preparation
To be indicated
14.1 special precautions concerning the transport of a substance or preparation outside the premises or at the premises of the manufacturer, importer or distributor,
14.2 additional information, if appropriate (e.g. reference to legislation on the transport of dangerous goods) .7)
15. Information on legislation regarding the substance or preparation
Reference shall be made to the legislation relating to the substance or preparation.
16. Further information on the substance or preparation
To be indicated
16.1 date of issue of the safety data sheet, unless otherwise specified,
16.2 information relevant to human safety and health (e.g. instructions for training, recommended use of the substance or preparation, recommendations for limiting the use of the substance or preparation),
16.3 information on the contact point of the manufacturer or importer,
16.4 information on the sources of data used in compiling the safety data sheet.
