DECLARATION
of 27 January 2014
amending Decree No. 100 / 2012 Coll., on the Prescription and Treatment of Medical Devices
The Ministry of Health provides pursuant to § 17 paragraph 1 of Act No. 123 / 2000 Coll., on Medical Devices and on the amendment of certain related laws, as amended by Act No. 375 / 2011 Coll.:
Decree No. 100 / 2012 Coll., on the prescribing and handling of medical devices, is amended as follows:
1. In Paragraph 2 (1), the words "the voucher issued at the request of a patient who intends to use it in another Member State shall be added at the end of the text in point (b)."
2. In Article 2 (1) (g), the words "the voucher issued at the request of a patient intending to use it in another Member State shall be inserted after the word" nametag "; the words" the voucher issued at the request of a patient who intends to use it in another Member State "shall also indicate the professional qualifications and contact details, which shall be e-mail, telephone or fax indicating the international choice and the indication" Czech Republic "and."
3. In Paragraph 3 (2), the words "the voucher issued at the request of a patient who intends to use it in another Member State shall be added at the end of the text in point (a)."
4. In Article 3 (2) (f), the words "the voucher issued at the request of a patient intending to use it in another Member State shall be inserted after the word" nameplate "; the words" the voucher issued at the request of the patient which intends to use it in another Member State "shall also indicate the professional qualifications and contact details, which shall be e-mail, telephone or fax indicating the international choice and the indication" Czech Republic "and."
Efficacy
This decree shall take effect on the 15th day following its publication.
