DECLARATION
of 22 August 2014
on model documents under the Act on Citizenship of the Czech Republic
The Ministry of Interior provides pursuant to § 79 of Act No. 186 / 2013 Coll., on Citizenship of the Czech Republic and amending certain laws (Act on Citizenship of the Czech Republic) ("the Act '):
Charter on the acquisition of citizenship of the Czech Republic by grant
The model of the Act on the acquisition of citizenship of the Czech Republic is set out in Annex 1 to this Decree.
Charter on Acquisition of Citizenship of the Czech Republic by Declaration
Models of the Act of Acquisition of Citizenship of the Czech Republic by Declaration are set out in Annex 2 to this Decree.
Evidence of the disappearance of Czech citizenship
The models of the document of the expiry of citizenship are set out in Annex 3 to this Decree.
Certification
Models of the certificate according to § 43 of the Act, the certificate according to § 43 of the Act, in which, upon request, information is given as to how, and under what legislation, a natural person of Czech or Czechoslovak nationality has acquired or ceased, or also that at a certain date or at a certain time he was a Czech national citizen or a Czechoslovak national citizen, or that the natural person is a national citizen of the Czech Republic from a certain date continuously until the date of issue of the certificate, and the certificates provided for in § 48 of the Act are set out in Annex 4 to this Decree.
Forms
(1) The forms of documents the models of which appear in Annexes 1 to 4 to this Order are strictly negotiable forms with security features against their misuse, falsification and alteration. Each form shall bear a letter identifying the series and a six-digit number of each series from 000001 to 999999. After the number of the series has been exhausted, it shall be followed for each series form determined by the addition of an additional identical letter indicating the series and a six-digit number from 000001 to 999999.
(2) The forms are printed on free A4 paper of 100% pulp, 110 g / m2, colour PANTONE 365, without optical brightener with full chemical protection.
(3) In addition to the full chemical protection of the paper, the hedge elements of the forms are owned by a unique and unmistakable continuous watermark of the lion in the shield and of the leaves.
Repeal
Decree No. 425 / 2013 Coll., on model documents under the Act on Citizenship of the Czech Republic is hereby repealed.
Efficacy
This Decree shall enter into force on 1 January 2015.
