amending Government Regulation No. 201 / 2010 Coll., on the method of recording accidents, reporting and sending an accident record

Government Decree No. 201 / 2010 Coll., on the method of recording accidents, reporting and sending an accident record, is amended as follows:
1. footnote 2 shall read:
"(2) Regulation (EC) No 1338 / 2008 of the European Parliament and of the Council of 16 December 2008 on Community statistics on public health and health and safety at work. Commission Regulation (EU) No 349 / 2011 of 11 April 2011 implementing Regulation (EC) No 1338 / 2008 of the European Parliament and of the Council concerning Community statistics on public health and safety and health at work as regards statistics on accidents at work. ';
2. In Paragraph 2 (1), the words "as set out in Annex 3 to this Regulation 'shall be added at the end of the text in point (g).
3. in Article 2 (1), the following point (h) is inserted after point (g):
"(h) a description of the accident,"
Points (h) to (l) shall be renumbered as points (i) to (m).
4. In Paragraph 2 (3), the word "copy 'is replaced by" certified copy'.
5. Article 4 (1) shall be replaced by a dot at the end of point (e) and point (f) shall be deleted, including footnote 14.
6. In Article 6, at the end of point (d), the comma is replaced by a dot and point (e) is deleted.
7. In Article 7, at the end of point (d), comma is replaced by a dot and point (e) is deleted.
8. The following Section 7a is inserted after Section 7, including footnote 16:
The employer shall send, for the purposes of the liquidation of claims, a record of the accident at work or occupational disease and of the organisational unit of the insurance undertaking with which the insurance is insured for this purpose (16).
16) Paragraph 8 (c) of Decree No. 125 / 1993 Coll., laying down the conditions and rates of statutory insurance of the employer's liability for damage caused by accidents at work or occupational disease. '
9. in Article 8 (2) (a):
"(a) hospitalisation by accident at the affected staff member has exceeded five consecutive days and has been completed following the dispatch of the accident record;"
10. in Paragraph 8 (2) (c), the word "or" shall be deleted;
11. in Paragraph 8, the dot is replaced by a comma at the end of paragraph 2 and the following point (e) is added:
"(e) a further incapacity for work has been created and terminated following the same accident at work."
12. in Paragraph 8 (3), the comma at the end of point (d) shall be replaced by a dot and point (e) shall be deleted;
13. in Paragraph 8 (4):
"(4) The employer shall send, for the purposes of the liquidation of claims, a record of the accident - notification of the changes and the organisational component of the insurance undertaking for which the insurance is insured for that purpose, for the purpose of the statutory insurance of the employer's liability for damage caused by an accident at work or occupational disease and for a fatal accident at work. 16). '
14. In Article 8, the following paragraph 5 is inserted after paragraph 4:
"(5) Where there has been a change in the assessment of the source or cause of the accident, the nature of the accident and, where appropriate, other factors affecting the processing and content of the accident record - notification of changes, the employer shall forward one copy of the accident record - notification of changes to the accident suffered by the staff member and in the event of a fatal accident at work to his family members. ';
Paragraph 5 shall become paragraph 6.
15. Annexes 1 and 2 shall read as follows:
