DECLARATION
of 18 May 2007
on the model of the driver's memory card
The Ministry of Transport provides pursuant to § 137 (2) of Act No. 361 / 2000 Coll., on road traffic and on amendments to certain laws (Road Traffic Act), as amended by Act No. 411 / 2005 Coll. and Act No. 226 / 2006 Coll., for the implementation of § 110a (7) of this Act:
Subject matter
This decree sets out the model of the driver's memory card in accordance with the directly applicable European Union1.
Driver memory card
(1) Driver memory card is a 54 x 86 mm plastic card, white and equipped with a memory chip.
(2) The individual data on the driver's memory card are given in the Czech language, in the letters of the Latin alphabet, except for the owner's handwritten signature of the driver's memory card.
(3) The following information is given on the front of the driver's memory card:
(a) the distinguishing mark of the Czech Republic "CZ," printed in white in a blue rectangle, placed in a circle of 12 yellow stars,
(b) the inscription "CARTA DEVICE" in capital letters in Czech;
(c) the words "CARTA DEVICE" in the Czech language and in the languages of the other Member States of the European Union, which are printed in grey as part of the front of the memory card;
(d) the designation "G2" in large letters in blue.
(4) On the front of the driver memory card, the following information is given under the code numbers:
1st surname,
2. name,
3. date of birth,
4a. the starting date of the card,
4b. the expiry date of the card,
4c. the name of the issuing authority;
4d. administrative number,
5a. driving licence number,
5b. card number,
6. photographs,
7. driver's signature.
(5) The following information is given on the back of the driver's memory card:
(a) the inscription "CARTA DEVICE" in capital letters in Czech;
(b) an explanation of the individual data referred to in paragraph 4, including an indication of the relevant code number;
(c) instructions on the procedure in the event of the driver's memory card being found;
(d) under the chip is the designation of the national identifier "e8" made in blue.
(6) The data referred to in paragraph 4 referred to in point 4d shall not be completed on the driver's memory card.
(7) The model of the driver's memory card is set out in the annex to this decree.
Efficacy
This decree shall take effect on 1 July 2007.
