setting out the application forms for the appointment of a lawyer and the complaint forms for a one-off legal advice
The Ministry of Justice provides, pursuant to § 18a (4), § 18b (2) and § 18c (4) of Act No. 85 / 1996 Coll., on the Advocacy, as amended by Act No. 258 / 2017 Coll.:
Form models
(1) A request for legal advice pursuant to Article 18a of the Law on Advocacy shall be made on a form the formalities and model of which are set out in Annex 1 to this Order.
(2) The complaint for a one-off legal advice pursuant to Article 18b of the Law on Advocacy shall be submitted on a form the formalities and model of which are set out in Annex 2 to this Order.
(3) A request for legal service free of charge for a natural person whose income and property ratios justify it, and a request for legal service free of charge for a legal person whose income and property ratios justify it, pursuant to the first sentence of Paragraph 18c (4) of the First Law on Advocacy, shall be made on a form the formalities and model of which are set out in Annexes 3 and 4 to this Decree.
(4) The application for legal service for a payment to a natural person for reasons other than the income and property ratios of that natural person and the application for legal service for a payment for a legal person for reasons other than the income and property ratios of that legal person, pursuant to the second sentence of Paragraph 18c (4) and Article 23 (5) of the Law on Advocacy, shall be submitted on forms the models of which appear in Annexes 5 and 6 to this Order.
Repeal
Decree No. 275 / 2006 Coll., laying down the method for determining the income and property ratios of the applicant for the appointment of a lawyer by the Czech Bar Association for the provision of legal services free of charge or for reduced remuneration, is hereby repealed.
Efficacy
This Regulation shall enter into force on 1 July 2018.
