DECLARATION
of 5 March 2012
laying down models for diplomatic and service passports and forms for applications for their issue
The Ministry of Foreign Affairs provides pursuant to § 5 (5) (b) and § 20 (8) of Act No. 329 / 1999 Coll., on Travel Documents and amending Act No. 283 / 1991 Coll., on Police of the Czech Republic, as amended, (Act on Travel Documents), as amended by Act No. 136 / 2006 Coll., Act No. 140 / 2008 Coll., Act No. 197 / 2010 Coll. and Act No. 424 / 2010 Coll.:
Model diplomatic and service passports
(1) The model of the diplomatic passport with machine-readable data and the biometric data carrier is set out in Annex 1 to this Decree.
(2) The model of the service passport with machine-readable data and the data carrier with biometric data is set out in Annex 2 to this Decree.
(3) The machine-readable data are recorded in the designated zone at the bottom of the diplomatic and service passports listed in Annexes 1 and 2 to this Regulation.
(4) The biometric data carrier shall be located on the other side of the diplomatic and service passports listed in Annexes 1 and 2 to this Regulation and shall be legible when those passports are opened.
(5) The unabridged form of the name or surnames, where appropriate, shall be entered in the official records on page 3 of the diplomatic and service passports listed in Annexes 1 and 2 thereto.
(6) On pages 6 and 7 of the diplomatic and service passports listed in Annexes 1 and 2 to this Regulation, a register is given (1) with the translation of the names of the relevant data entered in these passports in the German, Italian, Spanish, Portuguese, Dutch, Swedish, Danish, Finnish, Greek, Irish, Slovak, Slovenian, Polish, Hungarian, Maltese, Lithuanian, Estonian, Latvian, Russian, Chinese and Arabic languages.
Models of application forms
(1) The model of the application form for the issue of a diplomatic passport with machine-readable data and a biometric data carrier is set out in Annex 3 to this Regulation.
(2) The model of the application form for the issue of a service passport with machine-readable data and a biometric data carrier is set out in Annex 4 to this Decree.
Transitional provisions
(1) The forms of applications for the issue of a diplomatic and service passport with machine-readable data and a data carrier with biometric data in accordance with the models set out in Annexes 7 and 8 to Decree No 495 / 2006 Coll., as effective by the date of entry into force of this decree, may be used until 31 December 2012 at the latest.
(2) Diplomatic and service passports with machine-readable data and a data carrier with biometric data in accordance with the models set out in Annexes 2 and 4 to Decree No 495 / 2006 Coll., as effective until the date of entry into force of this decree, may be issued no later than 31 December 2012.
Repeal
The following shall be deleted:
1. Decree No. 495 / 2006 Coll., on the establishment of models of diplomatic and service passports and forms of applications for extradition.
2. Decree No. 172 / 2008 Coll., amending Decree No. 495 / 2006 Coll., laying down models for diplomatic and service passports and forms for applications for extradition.
Efficacy
This decree shall take effect on the day of its publication.

1) Annex 1 to the Resolution of the Representatives of the Governments of the Member States meeting within the Council of 23 June 1981 on the introduction of a uniform passport model, as amended by Resolutions of 30 June 1982, 14 July 1986, 10 July 1995 and 8 June 2004.
