DECLARATION
of 23 April 2014
on the formalities for the certification of employees of social security institutions and of sickness insurance authorities responsible for checking the performance of obligations imposed on employers and self-employed persons
The Ministry of Labour and Social Affairs provides, pursuant to § 127 (3) of Act No. 582 / 1991 Coll., on the organisation and implementation of social security, as amended by Act No. 64 / 2014 Coll., pursuant to § 199 (1) of Act No. 187 / 2006 Coll., on sickness insurance, as amended by Act No. 64 / 2014 Coll., and under Act No. 324 / 2025 Coll., on the compulsory contribution to products saving for old age and amending related laws (Act on the compulsory contribution to products saving for old age):
The document of the employee of the social security institution and of the sickness insurance institution responsible for checking the performance of the obligations imposed on employers and self-employed persons by the Act on the organisation and implementation of social security, the Social Security Insurance Act and the National Employment Policy Contribution Act, the sickness insurance law and the compulsory contribution to old age savings products shall contain:
(a) the designation "The document of the staff member responsible for inspection,"
(b) the name of the issuing authority;
(c) a large national character;
(d) a photograph of an employee meeting the requirements laid down for photographs under the Decree implementing the Civil ID Act and certain provisions of the Travel Documents Act and the Basic Register Act;
(e) the title, the name and, where appropriate, the names and surnames of the staff member;
(f) the personal number of the staff member;
(g) control authorisation expressed in words "The holder of this licence is entitled to carry out checks on the performance of the obligations imposed on employers and self-employed persons by Act No. 582 / 1991 Coll., on the organisation and implementation of social security, as amended, and Act No. 589 / 1992 Coll., on social security contributions and the contribution to the state employment policy, as amended, Act No. 187 / 2006 Coll., on sickness insurance, as amended, and Act No. 324 / 2025 Coll., on the compulsory contribution to old age savings products. ';
(h) the licence number;
(i) the date of issue of the licence;
(j) the official stamp of the issuing authority and the signature of the natural person authorised to issue the licence.
This Decision shall enter into force on 1 May 2014.
