GOVERNMENT REGULATION
of 8 March 2000
implementing Act No. 216 / 1991 Coll., on travel documents and travel abroad, as amended
The Government orders the implementation of Act No. 216 / 1991 Coll., on Travel Documents and Travel abroad, as amended by Act No. 150 / 1996 Coll. and Act No. 326 / 1999 Coll.:
§ 1
At the request of a citizen, the following particulars shall be entered on the travel document:
(a) in the travel document, the parent's name and surname, if different from the parent's surname, year of birth and sex of a minor under 15 years of age;
(b) the title of a graduate of a higher vocational school, (1) an academic degree, a status mark, another graduate degree of a university (hereinafter referred to as the title) or a scientific degree; (2) if a citizen receives more than one degree or a scientific grade, only one degree and one degree of a citizen's choice shall be registered.
§ 2
The following data may be entered in the travel document at the border crossing point:
(a) the year, month, day, hour and direction of crossing the national borders of the Czech Republic;
(b) the number of minors under 15 years of age accompanied by a parent if they are entered in his travel document;
(c) the name of the border crossing point.
This information shall be entered on the travel document if the citizen so requests.
§ 3
The travel document may make changes to the information on:
(a) a person under 15 years of age entered on the travel document by his or her parent, if the travel document is the passport, diplomatic passport, service passport or passport;
(b) the period of validity of the travel document, if this document is a travel document.
§ 4
The application for the issue of a travel document shall be accompanied by two sharp, non-printed photographs of the size 35 x 45 mm, made from the same negative on a smooth, glossy semi-carton or made by a device with simultaneous exposure to multiple lenses, showing the citizen in front of the front view with a face-to-chin height of at least 13 mm, without glasses with dark glass, except for blind and headgear, unless its use is justified by religious or medical reasons; However, the headgear shall not cover the facial part in such a way as to prevent the identification of the citizen.
§ 5
This Regulation shall enter into force on the day of its publication and shall expire on the day immediately preceding the date on which Act No. 329 / 1999 Coll., on travel documents and amending Act No. 283 / 1991 Coll., on the Police of the Czech Republic, as amended, (Act on travel documents).
