DECLARATION

amending Decree No. 400 / 2011 Coll., implementing the Civil ID Act and the Travel Document Act
According to § 26 (a) and (b) of Act No. 328 / 1999 Coll., on Civil Documents, as amended by Act No. 424 / 2010 Coll.:
Decree No. 400 / 2011 Coll., implementing the Civil ID Act and the Travel Document Act, is amended as follows:
1. Annexes 1 and 2 shall read:
"Annex No 1 to Decree No 400 / 2011 Coll.
Model of identity card with machine readable data and contact electronic chip
In the event that a citizen has made a declaration of entry into a partnership or partnership, the death of one of the partners, the death of one of the partners dead or the cancellation of a court decision, the model of the back of the identity card with machine-readable data and the contact electronic chip shall be as follows:

Transitional provision
The forms of the certificate of identity card issued pursuant to Decree No. 400 / 2011 Coll., as effective before the date of entry into force of this Order, may be used until 31 December 2018 at the latest.
Efficacy
This Decree shall take effect on 19 May 2014.
