of 21 December 2004
laying down the details of the organisation of the admission procedure for secondary school education
The Ministry of Education, Youth and Sports provides according to § 64 of Act No. 561 / 2004 Coll., on Pre-School, Basic, Medium, Higher Vocational and Other Education (Education Act):
ORGANISATION OF THE ACCEPTANCE PROCEDURE TO THE FIRST YEAR OF EDUCATION IN THE CENTRAL SCHOOL
Forms of application for education
(1) The following documents, or certified copies thereof, shall form part of the training application (hereinafter referred to as "the application," unless otherwise specified below:
(a) a certificate from the last two years in which the applicant has completed or has completed compulsory schooling or from the corresponding primary school years, even after completion of compulsory schooling;
(b) a certificate from the last two years in which the tenderer has completed or terminated basic education if the tenderer has obtained or received basic education before completing compulsory education;
(c) a certificate from the sixth and seventh grade of primary school, or from the fourth and fifth grade of primary school, if the applicant applies to the first grade of the six-year high school, or eight-year high school,
(d) a certificate from the last two years in which the applicant has performed or completed compulsory schooling if the applicant has applied for the first year of the eight-year conservatory training programme;
(e) documents relating to the acquisition of secondary education with a certificate, if the applicant applies to the extension studies1),
(f) proof of obtaining the relevant secondary education degree if the applicant applies for a short study for receiving secondary education with a listemym) or for a short study for receiving secondary education with a graduate examination (3);
(g) a medical assessmenton medical fitness (4), provided that, under the Government Regulation on the education system (10), the admission of a candidate to education is a prerequisite for fulfilling the applicant's medical fitness conditions for the relevant field of education;
(h) a decision on a health handicap under a special legislature5) if the applicant is a disabled person;
(i) the recommendation of the educational advisory establishment containing support measures for the necessary adjustments to the admission procedure in the case of applicants with special educational needs;
(j) proof of completion of compulsory schooling in the case of a candidate who has completed or terminated compulsory schooling in a foreign school, issued by a foreign school;
(k) a certificate of recognition of the equivalence of a foreign certificate issued by a foreign school or of a decision to recognise the validity of a foreign certificate where the appropriate degree of education is required for admission.
(2) The application also includes documents relating to the criteria of the admission procedure laid down by the Director of the Central School6), to be determined by the School Director, such as evidence of results in professional competitions, publishing activities, obtained sub-qualifications6b) or further education in the school 7).
(3) The application may not include the documents or certified copies referred to in points (a) to (d) of paragraph 1, provided that the classification from the relevant years on the application is verified by the school in which the applicant has completed or has completed compulsory schooling.
(4) The final certificate certifying the acquisition of secondary education with a certificate or secondary education with a final examination shall be submitted by the applicant immediately after its issue, unless it is issued at the time of the application, no later than the date on which he becomes a pupil of the school7a).
(5) The final certificate certifying the completion of the fifth year of the primary school, the seventh year of the primary school, the completion of the primary school before completion of compulsory schooling or, where appropriate, the proper completion of compulsory schooling, shall be submitted by the applicant immediately after its issue, unless it is issued at the time of application, no later than the date on which he becomes a pupil of the school 7a).
(6) In the case of documents drawn up in a language other than the Czech language, the tenderer shall attach an official translation to the Czech language, unless the principal of the school informs the applicant that the official translation is not required. In the case of documents drawn up in the Slovak language, translation into the Czech language is not required.
Announcement of the first round of the admission procedure
(1) The school director shall announce the first round of the admission procedure to the fields of education with a talent test by 31 October at the latest, to the other fields of education by 31 January, by publishing in a publicly accessible place at the school and in a way that allows remote access.
(2) In the case of a school or a field of education in the school register after 31 January of the relevant year, the school director may declare the first round of the admission procedure by another deadline, but not later than 31 August.
Application procedure
(1) The admission examination may be carried out by means of:
(a) examinations commissioned by the school director;
(b) centrally entered and evaluated tests; or
(c) a combination of the methods referred to in (a) and (b).
(2) The acceptance test referred to in paragraph 1 (a) shall take place in the first round of the admission procedure on working days from 22 to 30 April.
(3) The Director of the School may pre-rank the examination referred to in paragraph 1 (b) up to five working days before the deadline for the examination referred to in paragraph 1 (a).
(4) Talent examination in the field of education with talent examination in the first round of the admission procedure takes place on working days from 2 to 15 January.
(5) Talent testing at the Conservatory in the first round of the admission procedure shall take place on working days from 15 to 31 January.
(6) The days of the examination referred to in paragraphs 1 to 5 shall be determined by the school director.
(1) In the event that a joint recruitment procedure takes place in several fields of education, the school director, together with the announcement of the admission procedure, shall publish this fact for the first round of the admission procedure as part of the notification provided for in Section 2.
(2) In the event of the announcement of further rounds of the admission procedure, the date by which applications for training may be submitted at the latest shall be announced.
(3) The invitation to take the entrance examination or the talent examination shall be sent by the principal of the school to the tenderer no later than 7 days before the examination.
(4) The invitation for the entrance examination or the talent test shall include information on the requirements for these examinations, the expected number of candidates admitted and the criteria of the admission procedure, as determined by the school head7b).
The school director shall ensure that applicants with special educational needs have adequate conditions in the recruitment procedure as recommended by the educational institution in accordance with the Education Act and the Decree governing the education of children, pupils and students with special educational needs.
The publication of the list of candidates admitted and the dispatch of decisions not to accept non-accepted candidates or legal representatives of non-adopted minors shall be subject to the deadlines set out in Article 3 (2).
(1) The Head of the School shall publish in a publicly accessible place at the school and in a manner that allows remote access, in accordance with the legislation on the protection of personal data (8),
(a) the ranking of tenderers according to the outcome of the evaluation of the admission procedure;
(b) the criteria for the admission procedure.
(2) The information referred to in paragraph 1 shall be published in the Official Journal of the European Union.
(a) within 3 working days of the last date determined by the school's principal for the entrance examination; or
(b) the entrance examination has not been established by the school director, after evaluation of the admission procedure, not later than the last day of the deadline referred to in § 3 (1) and in other rounds without undue delay in the first round.
(3) The order of tenderers shall be published under the registration number. The registration number shall be assigned to the candidate at the school at the latest on the day of the entrance examination or the talent examination before the start of the examination. In the event that the entrance examination is not held or is not required to take place in accordance with the criteria of the admission procedure, the applicant shall be informed in writing by the principal of the school at the same time as the information on the absence of the entrance examination.
(4) For the next round of the admission procedure, the applicant shall, at his request, issue the application form to the principal of the school who has announced the next round.
(1) The number of vacancies for appeal procedures shall be determined by the school director.
(2) The head of the school shall forward to the Regional Office, together with the appeal of the criteria of the admission procedure under which he followed the admission procedure.
Registration form
(1) The form of the entry note shall be accompanied by a grey copy of the leaves on each side of the form, a watermark with the motifs of the lip branches, a registration number containing a series of forms and at least a six-digit number of the form. The forms shall be drawn up on a sheet of 210 x 280 mm format with a tolerance of 3 mm.
(2) The model form for the entry note is set out in the Annex to this order. The place where the school's department identificator9) is indicated on the form by "IZO '.
(3) Registration of forms of registrations issued to individual tenderers is kept by the authority which issued them. The entry note is registered under the registration number for each applicant, which is issued in accordance with Section 60a of the Education Act. The Regional Office shall issue a entry note identifying the tenderer or legal representative of a minor on the basis of an identity check, or send it to the tenderer or legal representative of a minor at the request of a written request through the postal operator.
(4) At the time of receipt of the registration form, the secondary school or conservatory shall verify that the information contained in the registration form corresponds to the information in the application form.
(5) In the event of loss or destruction of the registration, the authority which issued it shall issue a replacement registration, upon written request, without undue delay. The application for a replacement registration card shall include an affidavit from the applicant or the legal representative of the minor applicant that the original registration card has neither applied nor applied in the secondary school; The signature of the tenderer shall be part of the honorary declaration of the legal representative of the minor tenderer. The replacement entry note shall be marked "REFERENCE 'before the heading of the entry note. Paragraphs 1 to 4 shall apply mutatis mutandis to the replacement entry note.
ORGANISATION OF THE ACCESSION PROCEDURE TO THE HIGH ANNUAL
The application shall include the documents or certified copies referred to in Article 1 (1) (g) to (i) and (k) and (2), as well as the documents of the applicant for prior training, for determining the form, content and criteria of the admission procedure and for determining the year to which the tenderer will be classified.
COMMON AND FINAL PROVISIONS
This decree also applies to admission to education in the Conservatory.
The following shall be deleted:
1. Decree No. 10 / 1997 Coll., on the admission of pupils and other applicants to study in secondary schools established by the State.
2. Decree No. 469 / 2001 Coll., amending Decree No. 10 / 1997 Coll., on the admission of pupils and other applicants to study in secondary schools established by the State, and repealing certain legislation issued under the jurisdiction of the Ministry of Education, Youth and Sports.
3. Decree No. 160 / 2003 Coll., amending Decree No. 10 / 1997 Coll., on the admission of pupils and other applicants to study in secondary schools established by the State, as amended by Decree No. 469 / 2001 Coll.
This Decree shall take effect on 1 January 2005.
