DECLARATION
of 26 March 2014
on the model of service card staff of the Czech Trade Inspection Agency
The Ministry of Industry and Trade provides pursuant to § 4 of Act No. 64 / 1986 Coll., on Czech Trade Inspection, as amended by Act No. 240 / 1992 Coll., Act No. 22 / 1997 Coll., Act No. 145 / 2000 Coll. and Act No. 64 / 2014 Coll.:
The model of the service card of the Czech Trade Inspection Staff responsible for carrying out its inspection tasks is set out in the annex to this Order.
This decree shall take effect on the first day of the calendar month following its publication.

Model of service card staff Czech Trade Inspection
1. Description
a) The front page of the service card shows the name of the inspection body - Czech Trade Inspection. The second line shall bear the mark of the service card. The third line shows the personal number of the employee, the fourth line contains the name and the fifth line contains the surname. The sixth line shows the position of the employee. The seventh line shall show the competent inspector. In the lower part, the words "Service card entitles its holder to carry out control activities under the laws governing the activities of the Czech Trade Inspection Office."
b) On the reverse of the service card, the name of the inspection body - Czech Trade Inspection Office is given on the first line. The following is followed by the text "The Czech Trade Inspection Card is a public instrument. Counterfeiting or amending an authentic instrument is criminal. The professional card finder is obliged to submit it to the nearest office of the Police of the Czech Republic or to any of the inspectors of the Czech Trade Inspection Office. Abuse of the Czech Trade Inspection Service Card is criminal. 'The lower part includes the date of issue of the licence, the stamp and the employer's signature.
(c) The inspector's card shall be 99 × 69 mm and shall be stored in a plastic package. On the reverse side of the lower part is a hologram with the inscription DOI. The photograph of the employee is colour-coloured with dimensions 3,5 × 4,5 cm and is placed on the RS. The requirements for the technical execution of the photograph shall comply with the technical requirements for photographs for the issue of the identity card.
