DECLARATION

amending Decree No 642 / 2004 Coll., implementing Act on Civil Documents and the Act on Travel Documents, as amended, and Decree No 417 / 2006 Coll., amending Decree No 642 / 2004 Coll., implementing Act on Civil Documents and the Act on Travel Documents, as amended by Decree No 299 / 2006 Coll.
The Ministry of Interior provides pursuant to § 5 (6) (a) of Act No. 329 / 1999 Coll., on Travel Documents and amending Act No. 283 / 1991 Coll., on Police of the Czech Republic, as amended, (Act on Travel Documents), as amended by Act No. 136 / 2006 Coll. and Act No. 106 / 2007 Coll.:
Amendment of Decree No. 642 / 2004 Coll., implementing the Act on Civil Documents and the Act on Travel Documents, as amended by Decree No. 299 / 2006 Coll. and Decree No. 417 / 2006 Coll.
Decree No. 642 / 2004 Coll., implementing the Act on Civil Documents and the Act on Travel Documents, as amended by Decree No. 299 / 2006 Coll. and Decree No. 417 / 2006 Coll., is amended as follows:
1. the following Section 12a is inserted after Section 12:
Technical method of registration of citizens under 10 years of age in the travel document
(1) The particulars referred to in Section 7 (1) (b) of the Travel Documents Act are entered in the travel document under the heading, except for passports listed in Annex 8 and Annex 9.
(2) The particulars referred to in paragraph 1 shall be entered on the passport referred to in Annex 8 on a self-adhesive label affixed to page 4. If this page is filled with records, the label shall be affixed to the next next available page intended to indicate the visa.
(3) The entry of a citizen under 10 years of age in a travel document with machine-readable data and a data carrier with biometric data shall be requested on the application form set out in Annex 16.
(4) The particulars referred to in paragraph 1 shall be entered on the passport referred to in Annex 9 on a self-adhesive label affixed to page 29. If this page is filled with records, the label shall be affixed to the nearest preceding free page intended to indicate the visa.
(5) The model of the self-adhesive label is set out in Annex 17. "
2. In Annex No 1, after the words "Annex No 1 to Decree No 642 / 2004 Coll. ', the title of the Annex" Model of the ID card with machine-readable data' is inserted.
3. In Annex No 2, after the words "Annex No 2 to Decree No 642 / 2004 Coll. ', the title of the Annex" Model of the ID card without machine-readable data' is inserted.
4. In Annex 3, the title of the Annex "Model of the certificate of identity card 'is inserted after the words" Annex 3 to Decree No 642 / 2004 Coll.'.
5. In Annex No 4, after the words "Annex No 4 to Decree No 642 / 2004 Coll. ', the title of the Annex" Model form of the certificate of amendment to the particulars entered on the ID' is inserted.
6. In Annex 5, Annex 5 to Decree No 642 / 2004 Coll. 'is inserted after the words "Annex 5 to Regulation No 642 / 2004 Coll.'.
7. In Annex 6, after the words "Annex 6 to Decree No 642 / 2004 Coll. ', the title of the Annex" Model of the application form for the issue of a card without machine-readable data' is inserted.
8. In Annex 7, the title of the Annex "Size of the head of the person displayed, including its location 'is inserted after the words" Annex No 7 to Decree No 642 / 2004 Coll.'.
9. In Annex No 8, the title of the Annex "Model of passport with machine-readable data and with biometric data carrier 'is inserted after the words" Annex No 8 to Decree No 642 / 2004 Coll.'.
10. In Annex 9, the title of the Annex "Model passport without machine-readable data and without biometric data carrier 'is inserted after the words" Annex 9 to Decree No 642 / 2004 Coll.'.
11. In Annex No 10, the title of the Annex to the Annex "Model of passport 'is inserted after the words" Annex No 10 to Decree No 642 / 2004 Coll.'.
12. In Annex No 11, the title of the Annex "Model of another travel document issued under an international contract 'is inserted after the words" Annex No 11 to Decree No 642 / 2004 Coll.'.
13. Annex 12 shall read as follows:

16 In Annex No 15, the title of the Annex "Model forms of applications for the issue of other travel documents issued under international agreements (travel notes and cards) 'is inserted after the words" Annex No 15 to Decree No 642 / 2004 Coll.'.
16. Annex No 16 shall read as follows:

18. The following Annex 17 is added:

Transitional provisions
1. The forms of applications for the issue of a passport without machine-readable data and without a data medium with biometric data and of requests for changes to or additions to the data in the travel document issued in accordance with the models of these forms provided for in Decree No 642 / 2004 Coll., as effective until the date of entry into force of this decree, may be used until 31 December 2010 at the latest.
2. The travel licence application forms issued in accordance with the model of this form provided for in Decree No 642 / 2004 Coll., as effective until the date of entry into force of this Order, may be used until 31 December 2012 at the latest.
Amendment to Decree No. 417 / 2006 Coll., amending Decree No. 642 / 2004 Coll., implementing the Civil ID Act and the Travel Documents Act, as amended by Decree No. 299 / 2006 Coll.
In point 2 of Article II of Decree No. 417 / 2006 Coll., amending Decree No. 642 / 2004 Coll., implementing the Civil ID Act and the Travel Documents Act, as amended by Decree No. 299 / 2006 Coll., "until 31 December 2009 'is replaced by" until 31 December 2012'.
EFFECTIVE
This Decree shall take effect on 1 March 2008, with the exception of Article I (13), which shall take effect on 1 April 2008.
