DECLARATION
Ministry of Health

amending and supplementing Decree No. 426 / 1992 of the Ministry of Health of the Czech Republic Coll., on the reimbursement of medicines and medical devices, as amended by Decree No. 150 / 1994 Coll. and Decree No. 198 / 1995 Coll.
The Ministry of Health shall, following a conciliation procedure with representatives of the Ministry of Finance, General Health Insurance of the Czech Republic, other health insurance companies, contractual health institutions, professional organisations established by the Act and professional scientific companies pursuant to Section 11 (4) of Act No. 20 / 1966 Coll., on the care of the health of the people, as amended by Act No. 548 / 1991 Coll., and by Act No. 59 / 1995 Coll.:
Decree of the Ministry of Health of the Czech Republic No. 426 / 1992 Coll., on the reimbursement of medicines and medical devices, as amended by Decree No. 150 / 1994 Coll. and Decree No. 198 / 1995 Coll., is amended as follows:
1. the second sentence of Paragraph 1 (2) (a) reads: "The list of medicines which also contains an interpretation of the terms of symbols, the expertise of doctors required to prescribe individual medicines and the indication of the restriction of medicines is set out in the Annex which forms part of this decree."
2. the Annex to Decree No. 198 / 1995 Coll. is amended as follows:
(a) in the part in which abbreviations are given in the list of medicinal products
(aa) under (c) for the symbol "O", a semicolon shall be added at the end of the text and the following words shall be added: "The payment by the health insurance undertaking may be linked to an assessment by a professional doctor or to an indication restriction on the list ';
(ab) for point (d), the title shall be "medical expertise and the designation of specialised workplaces";
(ac) for (d) at the end of the text, the following entries are added:

(c) the following amendments and additions are made under ATC heading "List of medicines":
1. the words "for calcium content" shall be added to the column "ADDITIONAL NAMES";
2. in the column "ADDITIONAL NAMES," the words "potassium content" shall be added to the column "3" shall be amended to "1,5" and in the column "DF" shall be amended to "GM";
(3) B01AD04 and B06AA are deleted from the "4.100,00" column and in the LIMIT column the symbol "K" is replaced by the symbol "Z";
4. C07AB04, C07AB05 and C07AB07 in the OCCUPATION column, the entry "7,90 'shall be changed to" 6,05', the symbol "L 'shall be deleted in the LIMIT column and the abbreviation" INT' shall be deleted in the LIMIT column;
5. G02AD01, G02AD02 part., G02AD02 vag., G02AD04 and G02AD05 in LIMIT column, the symbol "K 'is replaced by the symbol" K / P' and the abbreviation "ICU 'is deleted in the LIMIT column;
6. G03HA01 p. o. in the LIMITATION column, the abbreviation "URN" is added;
7. G03HB01 cyproterone acetate / ethinyl estradiol in the LIMITATION column is added the abbreviation "END";
8. H01AC01 in the OCCC column, the entry "818,30 'is added, the symbol" Z' is changed in the LIMIT column to the symbol "L 'and the abbreviation" J5' is added to the OCCC column;
9. H05BA01 inhal. and H05BA01 parent. and H05BA03 in the LIMITATION column, the abbreviation "END" is added;
