GOVERNMENT REGULATION
of 24 November 2004
on the establishment of application forms for individual and aggregate export authorisations and applications for international import certificates for dual-use goods and technology
The Government hereby orders, pursuant to § 7 (4), § 14 (9) and § 24 of Act No. 594 / 2004 Coll., implementing the European Community regime for the control of exports of dual-use goods and technology ("the Act '):
Subject matter
This Regulation (1) regulates, following the directly applicable regulation of the European Community2)
(a) the application form for individual export authorisation and summary export authorisation pursuant to Article 7 (4) of the Act;
(b) the application form for an international import certificate pursuant to Section 14 (9) of the Act.
The form set out in Annex 1 to this Regulation shall be used for the submission of an application for individual export authorisation and a summary export authorisation for dual-use goods and technology ("dual-use items').
The application for an international import certificate for dual-use items shall be made using the form set out in Annex 2 to this Regulation.
Efficacy
This Regulation shall enter into force on the day of its publication.

1) It is issued for implementation and within the limits of a law already adapted to the directly applicable provisions of the European Communities.
2) Council Regulation (EC) No 1334 / 2000 establishing a Community regime for the control of exports of dual-use items and technology, as amended.
