laying down the structure and format of the form for submitting notifications under the conflict of interest law
The Ministry of Justice provides, pursuant to § 12 (5) of Act No. 159 / 2006 Coll., on Conflict of Interests ("the Act '):
The structure and format of the form to be notified pursuant to Articles 9 to 11 and 12 (2) of the Act are set out in the Annex to this Decree.
This Decree shall take effect on 1 January 2007.
