DECLARATION

of 26 March 2014

on forms according to the law on addictive substances

According to Section 44b of Act No. 167 / 1998 Coll., on addictive substances and amending certain other laws, as amended by Act No. 74 / 2006 Coll., Act No. 141 / 2009 Coll. and Act No. 273 / 2013 Coll., ("the Act '):

§ 1

Form forms

(1) This Order sets out the form of the form

(a) applications for authorisation for the treatment of addictive substances and preparations pursuant to Article 8 (6) of the Act in Annex 1 to this Decree;

(b) applications for the modification of the authorisation for the treatment of addictive substances and preparations pursuant to Article 9 (10) of the Act in Annex 2 to this Decree;

(c) applications for revocation of the authorisation for the treatment of addictive substances and preparations referred to in Article 8 (11) of the Act in Annex 3 to this Decree;

(d) applications for inclusion in the list of persons, specifying their places of work, for whose activity no authorisation is required for the treatment of addictive substances and preparations containing them in accordance with Article 5 (7) of the Act, in Annex 4 thereto,

(e) requests to amend the information contained in the application for inclusion in the list of persons, with an indication of their places of work, for whose activity no authorisation is required for the treatment of addictive substances and preparations containing them in accordance with Article 5 (7) of the Act, in Annex 5 to this Decree,

(f) applications for authorisation to import addictive substances and preparations pursuant to Section 22 (2) of the Act in Annex 6 to this Decree;

(g) an estimate of the imports of addictive substances and preparations pursuant to Article 26 (1) (b) of the Act in Annex 7 to this Decree;

(h) applications for authorisation to export addictive substances and preparations pursuant to Section 22 (2) of the Act in Annex 8 to this Decree;

(i) applications for authorisation to export the macaque referred to in Article 25 (1) of the Act in Annex 9 to this Decree;

(j) an estimate of the production of addictive substances and preparations pursuant to Article 26 (1) (b) of the Act in Annex 10 to this Decree;

(k) the monthly report on the imports of addictive substances and preparations carried out pursuant to Article 26 (1) (c) of the Act in Annex 11 thereto,

(l) a monthly report on the exports of addictive substances and preparations carried out pursuant to Article 26 (1) (c) of the Act in Annex 12 to this Decree,

(m) the quarterly report on imports of macaquin pursuant to Section 30 of the Act in Annex 13 to this Decree;

(n) the quarterly report on the exports of macaquin pursuant to Section 30 of the Act in Annex 14 to this Decree,

(o) a summary of the annual reports on the production and consumption of addictive substances and preparations, on disposal, trade in them and on the state and movement of their stocks pursuant to Article 26 (1) (a) of the Act in Annex 15 to this Decree;

(p) Annex to Summary No I of the annual reports on the production and consumption of addictive substances and preparations, on disposal, trade in them and on the state and movement of their stocks - income from domestic suppliers in Annex 16 to this Decree,

(q) Annex, Summary II, annual reports on the production and consumption of addictive substances and preparations, disposal, trade in them and the state and movement of their stocks - supply to domestic customers in Annex 17 to this Decree,

(r) Annex to Summary III of the annual report on the production and consumption of addictive substances and preparations, on disposal, on transactions in them and on the state and movement of their stocks - details of the production in Annex 18 to this Regulation,

(s) Annex IV to the annual report on the production and consumption of addictive substances and preparations, on disposal, on transactions in them and on the state and movement of their stocks - to the own undertaking in Annex 19 to this Decree,

(t) Annex to Summary No V of the annual reports on the production and consumption of addictive substances and preparations, on disposal, on transactions in them and on the state and movement of their stocks - details of stocks in Annex 20 to this Regulation,

(u) Annex VI to the annual report on the production and consumption of addicts and preparations, on disposal, trade in them and on the state and movement of their stocks - disposal of waste and surplus and unused addicts and preparations in Annex 21 to this Regulation;

(v) an estimate of the cultivation of cannabis plants for research purposes, for the breeding of new varieties and for the conservation of genetic diversity pursuant to Article 26 (1) (b) of the Law in Annex 22 to this Decree;

(w) a summary of the annual reports on the cultivation of cannabis plants for research purposes, for the breeding of new varieties and for the conservation of genetic diversity pursuant to Article 26 (1) (a) of the Act in Annex 23 to this Decree;

(x) Annex to Summary No I of the annual report on the cultivation of hemp plants for research purposes, for the breeding of new varieties and for the conservation of genetic diversity - income from suppliers in Annex 24 to this Decree,

(y) Annex to Summary No II of the annual report on the cultivation of hemp plants for research purposes, for the breeding of new varieties and for the conservation of genetic diversity - the detail of the cultivation in Annex 25 to this Decree,

(z) Annex to Summary III of the annual report on the cultivation of hemp plants for research purposes, for the breeding of new varieties and for the conservation of genetic diversity - to the customers in Annex 26 to this decree.

(2) This Order also provides for the form

(a) Annex to Summary IV of the annual report on the cultivation of hemp plants for research purposes, for the breeding of new varieties and for the conservation of genetic diversity - details of the processing in Annex 27 to this Decree;

(b) Annex to Summary No V of the annual report on the cultivation of hemp plants for research purposes, for the breeding of new varieties and for the conservation of genetic diversity - detail of the destruction in Annex No 28 to this Decree;

(c) the statement by the Directorate-General for Customs on poppy sown areas and their estimates in accordance with Article 43 (6) (a) of the Act in Annex 29 to this Decree;

(d) the annual report by the Directorate-General for Customs on the cultivation of poppy seed pursuant to Article 43 (6) (b) of the Act in Annex 30 to this Decree;

(e) a report by the Directorate-General for Customs on sown areas of technical hemp plants and their estimates pursuant to Section 43 (6) (c) of the Act in Annex 31 to this Decree,

(f) the annual report by the Directorate-General for Customs on the cultivation of technical hemp pursuant to Article 43 (6) (d) of the Act in Annex 32 thereto.

§ 2

Repeal

Decree No. 125 / 2006 Coll., laying down forms of forms under Act No. 167 / 1998 Coll., on addictive substances and amending certain other laws, as amended, is hereby repealed.

§ 3

Final provision

This Decree was notified in accordance with Directive 98 / 34 / EC of the European Parliament and of the Council of 22 June 1998 laying down a procedure for the provision of information in the field of technical standards and regulations and of rules on information society services, as amended.

§ 4

Efficacy

This Decision shall enter into force on 15 April 2014.
