DECLARATION
of 4 October 2004
on the accreditation of equipment for the re-qualification of jobseekers and jobseekers
The Ministry of Education, Youth and Sports, in agreement with the Ministry of Labour and Social Affairs, provides, pursuant to § 108 (2) of Act No. 435 / 2004 Coll., on Employment, hereinafter referred to as "the Act":
Accreditation of equipment
(1) Accreditation of an establishment means the assignment of an establishment by the Ministry of Education, Youth and Sports to carry out retraining for a defined work activity under the training programme and to issue national retraining certificates.
(2) The condition for obtaining accreditation is:
(a) the submission of an application for accreditation with the correct and complete information referred to in paragraph 3 by a legal person or an undertaking by a natural person having, in the course of business or in the course of the activity of providing educational services;
(b) proof of adequate personnel, spatial, material and technical security of the training programme;
(c) evidence of the quality of the training programme in terms of determining the results of the training, defining the content and scope of the training, choosing the forms and methods of teaching and checking the results of the training in the retraining.
(3) The application for accreditation shall contain:
(a) identification data pursuant to Article 5 of the Act;
(b) a trade licence if the educational services are to be provided as a trade licence or any other document showing that the activity of the establishment involves the provision of educational services;
(c) an indication of the work to which the retraining will be ensured;
(d) the name, or names, surname and date of birth of the natural person responsible for the professional level of retraining and the evidence of his or her qualifications and professional practice (1);
(e) the name of the training programme;
(f) a profile of the graduate in which verifiable results of the training or work for which retraining is intended will be clearly, specifically and unambiguously indicated;
(g) conditions (input assumptions) for admission of a participant to the training programme;
(h) the organisational forms and methods of teaching, including the framework schedule of the hours of the teaching day, in the case of presentation;
(i) an indication of the manner and forms of verification of the knowledge and skills acquired;
(j) a curriculum containing a list of educational subjects with a specific hourly grant, the total number of hours of teaching divided into hours of theoretical preparation, the number of hours of practical training and the number of hours of examinations;
(k) syllables which include the content of the curriculum developed in detail in such a way as to make clear what the content of teaching is for the individual passwords of the curriculum, as well as the detailed results of education in each field, or blocks or modules where the curriculum is divided into them;
(l) the list of literature used in the programme and the recommended literature, for the distance form of teaching the model study material,
(m) a model of the retraining certificate to be issued after the successful completion of the training programme, indicating the name of the training programme, the work for which the retraining was carried out and the hourly teaching grants by subject;
(n) information on the expertise, qualifications, teaching and teaching practices of teachers for the teaching of individual subjects;
(o) information on the spatial, material and technical security of the training programme;
(p) proof of payment of the administrative charge (2).
(4) Accreditation is granted for a period of 3 years.
Efficacy
This decree shall take effect on the day of its publication.
