DECLARATION
of 16 December 2021
amending Decree No. 207 / 2001 Coll., implementing Act No. 301 / 2000 Coll., on Matrices, Names and Surnames, and amending certain related acts, as amended
According to § 96 (a) and (m) of Act No. 301 / 2000 Coll., on Matrices, Name and Surname and on the Amendment of Certain Related Acts, as amended by Act No. 312 / 2013 Coll. and Act No. 279 / 2019 Coll.:
Ordinance No. 207 / 2001 Coll., implementing Act No. 301 / 2000 Coll., on Matrices, Name and Surname, and on the Amendment of Certain Related Acts, as amended by Decree No. 230 / 2001 Coll., Decree No. 499 / 2001 Coll., Decree No. 566 / 2002 Coll., Decree No. 660 / 2004 Coll., Decree No. 539 / 2005 Coll., Decree No. 192 / 2006 Coll., Decree No. 352 / 2006 Coll., Decree No. 315 / 2010 Coll., Decree No. 417 / 2011 Coll., Decree No. 455 / 2012 Coll., Decree No. 445 / 2008 Coll., Decree No. 485 / 2009 Coll., Decree No. 485 / 2009 Coll., Decree No. 315 / 2010 Coll., Decree No. 415 / 2011 Coll.
1. The following Section 2b is inserted after Section 2a, including the title:
Model certificate
(1) Annex 5 provides a model of confirmation by the health service provider of the initiation of treatment for sex change.
(2) Annex 6 provides a model for confirmation by the health service provider of termination of treatment for sex change. '
2. In Annex 1, in the part of the Vysočina Region, in the column "Community of extended application ', in point 5." Jihlava', in the column "Matrix Office ', in point 7." Jamné', in the column "List of municipalities, urban areas, urban districts and military exits of the administrative district of the matrices', point 7 is added.
3. In Annex 1, in the part of the Vysočina Region, in the column "Community of extended application ', in point 5." Jihlava', in the column "Matrix Office ', point 17.
4. In Annex No 1, in the part of the Zlinsky region, in the column "Enlargement 'in point 13." Zlín, "in the column" Matrix Office, "point 4." 8. 11. In Annex 3, in the second column, the word "Thumb 'is replaced by the words" Hradec Králové'.
Transitional provision
Matrix Forms of the Marriage Questionnaire and the Protocol on Marriage pursuant to models 16 and 17 of Annex 2 to Decree No 207 / 2001 Coll., as effective before the date of entry into force of this Order, may be used until 31 December 2025 provided that the amendments referred to in points 8 to 10 of Article I are indicated.
Efficacy
This Decree shall take effect on 1 January 2022.
