DECLARATION

amending Decree No. 296 / 2004 Coll., implementing the Act on the registration of residents, as amended

According to § 25 (a) of Act No. 133 / 2000 Coll., on the registration of residents and birth numbers and on the amendment of certain laws (Act on the registration of residents):

Čl. I

Decree No. 296 / 2004 Coll., implementing the Act on the registration of residents, as amended by Decree No. 661 / 2004 Coll., Decree No. 299 / 2006 Coll. and Decree No. 84 / 2011 Coll., is amended as follows:

1. Annex No 1 shall read as follows:

"Annex No 1 to Decree No. 296 / 2004 Coll.

Model of the form "Login for permanent residence '.

2. In Annex No 2, after the words "Annex No 2 to Decree No. 296 / 2004 Coll. ', the title of the Annex" Model of the form "Proof of birth number' is inserted. '

3. Annexes 3 and 4 shall read as follows:

Transitional provision

The forms "Login card," "Request for data from the information system" and "Confirmation of change of residence" issued pursuant to Decree No. 296 / 2004 Coll., as effective before the date of entry into force of this Order, may be used no later than 31 December 2018.

Čl. III

Efficacy

This decree shall take effect on the 15th day following its publication.
