DECLARATION

of 7 November 2006

laying down models for diplomatic and service passports and forms for applications for their issue

The Ministry of Foreign Affairs provides pursuant to § 5 (6) (b) and § 20 (8) of Act No. 329 / 1999 Coll., on Travel Documents and amending Act No. 283 / 1991 Coll., on Police of the Czech Republic, as amended by Act No. 136 / 2006 Coll., hereinafter referred to as "the Act on Travel Documents':

§ 1

Model diplomatic and service passports

(1) The model of the diplomatic passport with machine-readable data for citizens under the age of 5 is set out in Annex 1 to this Decree.

(2) The model of the diplomatic passport with machine-readable data and the biometric data carrier is set out in Annex 2 to this Decree.

(3) The model of service passport with machine-readable data for citizens under the age of 5 is set out in Annex 3 to this Decree.

(4) The model of the service passport with machine-readable data and the data carrier with biometric data is set out in Annex 4 to this Decree.

(5) The machine-readable data shall be recorded in the lower part of the lower part of the diplomatic and service passports listed in Annexes 1 to 4 to this Order, where the data referred to in Section 6 (3) (a) and (b) of the Travel Documents Act are entered.

(6) The biometric data carrier shall be located on the other side of the diplomatic and service passports listed in Annexes 2 and 4 to this Regulation and shall be legible when the travel document is opened.

(7) On page 3 of the diplomatic and service passports listed in Annexes 2 and 4 to this Order, the information referred to in Section 7 (a) of the Travel Documents Act is entered.

(8) On pages 6 and 7 of the diplomatic and service passports listed in Annexes 1 to 4 to this Regulation, the register (1) lists the names of the relevant data entered in these passports in the German, Italian, Spanish, Portuguese, Dutch, Swedish, Danish, Finnish, Greek, Irish, Slovak, Slovenian, Polish, Hungarian, Maltese, Estonian, Latvian, Russian, Chinese and Arabic languages.

§ 2

Models of application forms

(1) The model of the application form for a diplomatic passport with machine-readable data for citizens under 5 years of age is set out in Annex 5 to this Decree.

(2) The model of the application form for a service passport with machine-readable data for citizens under 5 years of age is set out in Annex 6 to this Decree.

(3) The model of the application form for a diplomatic passport with machine-readable data and a biometric data carrier is set out in Annex 7 to this Decree.

(4) The model of the application form for a service passport with machine-readable data and a biometric data carrier is set out in Annex 8 to this Decree.

(5) The model of the application form for changes to the data and entry of the child in the diplomatic or service passport is set out in Annex 9 to this Decree.

§ 3

Repeal

The following shall be deleted:

1. Decree of the Ministry of Foreign Affairs No. 260 / 2000 Coll., implementing Act No. 329 / 1999 Coll., on Travel Documents and amending Act No. 283 / 1991 Coll., on Police of the Czech Republic, as amended, (Act on Travel Documents).

2. Decree No. 208 / 2003 Coll., amending Decree No. 260 / 2000 Coll., implementing Act No. 329 / 1999 Coll., on travel documents and amending Act No. 283 / 1991 Coll., on the Police of the Czech Republic, as amended, (Act on travel documents).

§ 4

Efficacy

This decree shall take effect on the day of its publication.
