implementing certain provisions of Act No. 332 / 2020 Coll., on the census of people, houses and flats in 2021 and amending Act No. 89 / 1995 Coll., on the State Statistical Service, as amended
Pursuant to Article 28 of Act No. 332 / 2020 Coll., on the census of people, houses and flats in 2021 and amending Act No. 89 / 1995 Coll., on the State Statistical Service, as amended, for the implementation of Sections 16 (2), 17 (3) and 22 (2):
Models of addition forms
The models of the census forms in paper form are set out in Annex 1 to this Decree.
Access to the electronic form and the means of verifying the authorisation to make it available
(1) The Czech Statistical Office will make the electronic form available through:
(a) the website www.onlinesciani.cz; and
(b) mobile applications.
(2) The authorisation to make the electronic form referred to in paragraph 1 available shall be verified:
(a) using an electronic identification device issued under a qualified electronic identification system (1);
(b) with a certified identity of the feeder in a way that can be logged into his data box (2); or
(c) by entering data on the date of birth and the number and type of electronically readable identification document kept in the population base register (3) or on the date of birth and on the identity card number kept in the civil ID registration agendation4).
Model of the census commissioner's card
(1) The census commissioner's card is in the form of a plastic card with dimensions 86 x 54 mm and contains the following information:
(a) the label and the logotype of the census, houses and apartments 2021,
(b) the title, the name and, where applicable, the names and surname of the adding Commissioner;
(c) the definition of the territorial scope of the adding Commissioner;
(d) the card number of the adding commissioner,
(e) the period of validity of the licence;
(f) a photograph of the adding Commissioner; and
(g) a protective element which makes it difficult to forge a licence.
(2) The model of the card of the adding Commissioner is set out in Annex 2 to this Order.
Efficacy
This Decision shall enter into force on 1 January 2021.

Model of the census commissioner's card
1. Description
a) The census commissioner's card is a white plastic card, size 86 x 54 mm.
(b) The front page of the census commissioner's card is provided in the upper 2 / 3 part of the graphic section of the census, houses and apartments 2021, in the lower right corner with the mark and the logotype of the census, houses and flats 2021. Under the graphic background, on the first line, the data are entered in the format of the academic title given before the name, name or name, surname, academic title given after the name. The last name shall be entered in capital letters. The text of the written data is in red (R: 235 G: 44 B: 64). On the second line, the text "Attractive Commissioner's Card 'appears. The text of the written data is written in blue (R: 36 G: 28 B: 135). In the left part there is digital processing of the photo of the adding commissioner 26 x 34 mm. The photograph of the census commissioner is colorful. The requirements for the technical processing of photographs comply with the technical requirements for photographs for the issue of the identity card. All texts, including written data, shall be drawn up in" Urban Grotesk' capitals.
c) The reverse side of the census commissioner's card is provided on the left side with a stripe with the graphic raster census, houses and apartments 2021, on the first to fifth lines the text "The census commissioner is entitled to perform the census of the people, houses and flats according to Act No. 332 / 2020 Coll., on census, houses and flats in 2021 and amending Act No. 89 / 1995 Coll., on the State Statistical Service, as amended. The card is non-transferable and is valid only in conjunction with the identity card. ', on the sixth line the text" COLLECTION PLACE' appears, on the seventh row the text "PROJECT NUMBER 'and on the eighth line the text" PROJECT VALIDITY' appears. In the field under the sixth line, the data on the definition of the territorial competence of the adding commissioner shall be entered under the text "COST NUMBER 'under the text" COST NUMBER' under the text "COST NUMBER ', the assigned number of the census commissioner shall be entered in the NNNNNNN format, and under the eighth line the term of validity of the licence shall be entered under the text" PERFORMANCE OF AN INSTANCE' under the text "DD.MM. - DD.MMM.RR. On the right side of the lower half of the licence are placed under the fifth line under the order the coloured logo of the Czech Post Office, p., under it the color logo of the Czech Statistical Office and under it the unique barcode in black. All texts, including written data, shall be in black in the capricious letters" Urban Grotesk '.
