on the extent of knowledge and other conditions for obtaining competence in certain fields of public health protection
According to Section 108 (1) of Act No. 258 / 2000 Coll., on the Protection of Public Health and on the Amendment of Certain Related Acts ("the Act '), the Ministry of Health provides for the implementation of Sections 19 (3), 59 (2), 60 (1) and (3) and 83e (2) of the Act:
Preliminary provisions
This decree regulates
(a) the content and scope of the courses in the field of special protective disinfection, disinsectisation and deratization, the extent of the knowledge necessary to protect public health in the performance of epidemiological activities and the knowledge to be demonstrated by a person in the proficiency test to carry out health risk assessments;
(b) the particulars of the application for a proficiency test in the field of special protective disinfection, disinsecting and exerting and the essential content of the test;
(c) the composition of the test committees, the conditions for carrying out the examination in the fields referred to in (a), the model certificates of competence and the model certificate of authorisation for the assessment of health risks.
Tests of competence and the content and scope of courses in the field of special protective disinfection, disinsecting and extermination
The content and scope of the courses to obtain knowledge for the performance of special protective disinfectant, disinfectant and exterminator, the completion of which is one of the conditions of professional activities in the field of special protective disinfectant, disinfectant and exterminator, are laid down in Annex 1.
(1) The application for a proficiency test shall state:
(a) the name and, where applicable, the name, surname, academic title, date of birth and address of the place of permanent residence or residence, if the person is not resident in the Czech Republic;
(b) length and content of professional experience;
(c) the field of activity for which the applicant requests a proficiency test.
(2) The application for a proficiency test under § 58 (1) to (3) of the Act must be accompanied by an officially certified copy of the proof of completion of the relevant professional course if the person has completed it.
(1) The proficiency test shall consist of a written and oral part; its content is a verification of the knowledge obtained to the extent set out in Annex 1.
(2) The condition for admission to the oral part of the proficiency test is the successful completion of a written test, meaning the correct answer to at least two thirds of the questions.
(3) The proficiency test is not public.
(1) The examination committee shall be of three members. It consists of the President and two other members appointed by the competent public health authority set up by the Commission (Section 60 (1) of the Act). Only a university-educated person active in the field of occupational hygiene or special protective disinfection, disinsecting and deratizing may be appointed as chairman of the Examination Board. The members of the Examination Board may be appointed only by university or secondary education persons engaged in the field of occupational hygiene or special protective disinfection, disinsection and deratization or professional direction for at least 5 years. Membership of the test committee is incompatible with the exercise of business in the field of special protective disinfection, disinsection and extermination.
(2) The examination committee shall evaluate separately the results of the proficiency test in its individual fields, which are special protective disinfectants, disinsections and exterminations. If the candidate has succeeded in all these fields, the examination committee shall evaluate the result of the proficiency test by saying "passed the proficiency test successfully." If the applicant has failed one or more proficiency tests in one or more fields, the panel shall evaluate the result by saying "he did not pass the proficiency test." The examination committee shall act by a majority vote.
The examination committee shall inform the candidate of its deadline, timetable and venue at least 14 days prior to carrying out the proficiency test.
(1) A report shall be drawn up by the chairman of the examination committee on the course of the proficiency test, indicating:
(a) the names and surnames of the members of the examination committee;
(b) the date and place of the written and oral part of the proficiency test and the time of their commencement and termination;
(c) the list of tenderers and the results of their evaluation;
(d) the date of completion of the Protocol.
(2) In addition to the protocol, written tests are submitted.
(3) The Protocol is signed by all members of the Examination Committee. The protocol shall be deposited at the appropriate regional health station. 1)
The model of the certificate of professional competence provided for in § 58 (1) to (3) of the Act is set out in Annex 5 to this Decree.
Test of knowledge necessary to protect public health in the performance of epidemiological activities
(1) The examination of the knowledge required to protect public health in the performance of epidemiological activity (2) consists only of the oral part. The extent of the knowledge necessary to protect public health in the performance of epidemiological activities is laid down in Annex 3.
(2) The examination committee and its composition and membership shall be subject to Paragraph 5 (1), with the exception that the qualifications of the chairman and members of the panel shall be based on activities in the field of epidemiology, nutrition and subjects of normal use or general and municipal hygiene carried out for at least 5 years.
Article 5 (2) and Article 7 (1) and (3) shall apply mutatis mutandis to the test and its operation.
Proficiency test to carry out health risk assessments
(1) The knowledge test necessary to carry out health risk assessments shall consist of:
(a) the written part in which the applicant is to demonstrate general knowledge of the principles of health risk assessment as set out in Part A of Annex 4 to this Regulation; and
(b) the oral part in which the applicant is to demonstrate general knowledge of the methods of assessing health risks and knowledge to the extent of the assessment of health risks of exposures in the specific areas referred to in Annex 4 to this Decree.
The applicant shall perform the written part of the examination before the oral examination. A condition for the acceptance of the oral part of the proficiency test to carry out the assessment of health risks shall be the successful performance of the written part, which shall consist in the correct answers to at least two thirds of the questions raised. The test is not public.
(2) The examination committee set up for the proficiency test to obtain authorisation for the assessment of health risks shall be of three members. It shall consist of the President and two other members. Only a university-educated person who has been active for at least 5 years in the health risk assessment section may be appointed as chairman of the Examination Board and its members.
(3) Paragraph 6 and 7 shall apply mutatis mutandis to the information of the applicant for the examination and the course of the examination. The test report shall be kept with the authorising officer for 10 years.
(4) The knowledge which the applicant must demonstrate during the proficiency test to obtain authorisation for carrying out health risk assessments is set out in Annex 4 to this Decree.
The model of the authorisation certificate provided for in Section 83e (1) of the Act is set out in Annex 6 to this Decree.
Efficacy
This decree shall take effect on 1 January 2001.

Scope of the knowledge necessary to protect public health in the performance of an epidemiological activity
1. The knowledge required to protect public health in terms of the prevention of alimentary diseases of persons performing work in the production and circulation of foodstuffs and in food services shall be:
(a) health requirements for persons carrying out the relevant activity;
(b) principles of personal hygiene at work;
(c) the principles of hygienically sound cleaning and disinfection, production technology, hygienically sound transport, storage and putting into circulation of epidemiological products;
(d) alimentary diseases and food poisoning (basic knowledge of their epidemiology and principles of prevention and spread),
(e) special hygiene issues according to the relevant work activity.
2. The knowledge needed to protect public health in terms of preventing early skin and sexual diseases of persons performing work in contact with the body of the customer, persons active in the establishments of massage, regeneration and recovery services, in the premises of hairdressers, pedicures, manicures, cosmetic care and the manufacture of cosmetic products shall be:
(a) health requirements for persons carrying out the relevant activity;
(b) basic hygiene principles at work;
(c) the principles of disinfection and sterilisation;
(d) early and skin diseases (basic knowledge of their epidemiology and prevention),
(e) the handling and storage of laundry;
(f) sexual diseases (basic knowledge of their epidemiology and prevention),
(g) special hygiene issues by working activity within the scope of the operating rules.
3. The knowledge required to protect public health in terms of the prevention of water-related diseases in persons coming into direct contact with water in the course of their working activities in water treatment plants and in the operation of water pipelines are:
(a) health requirements for persons carrying out the relevant activity;
(b) principles of personal hygiene at work;
(c) principles of hygienically sound operation and maintenance of water facilities;
(d) 1. basic knowledge of the causes, epidemiology and principles of prevention and spread of diseases in which water may be involved;
2. basic knowledge of the causes of water poisoning;
(e) special hygiene issues according to working activity within the scope of the operating rules of the water or water supply plant.

Forms of certificate of successful passing of the test of knowledge necessary to protect public health in the performance of epidemiological activities
Knowledge required to test competence to obtain authorisation for the assessment of health risks
A. General knowledge of the principles of health risk assessment:
1. The basis of the health risk assessment system and its link to epidemiology.
2. Hazardous noise properties and exposure / biological activity relationship.
3. Dangerous properties of non-ionising radiation and the relationship of exposure and biological effect.
4. Dangerous properties of chemicals and dust.
5. Dangerous properties of biological agents and relationship of exposure and biological effect.
6. Identification of hazards and selected chapters from toxicology, i.e. reproductive and developmental toxicity, neurotoxicity, genotoxicity, carcinogenicity, allergic hypersensitivity, hepatotoxicity, nephrotoxicity, immunotoxicity, local and systemic effects, acute and chronic effects of substances.
7. Dose (exposure) relationship and chemical response.
8. Estimation of exposure to biological agents in the environment, characterisation of health risks.
9. Estimation of exposure to noise in the environment and internal environment, characterisation of health risks.
10. Estimates of exposure to non-ionising radiation in the environment and the internal environment, characterisation of health risks.
11. Estimation of exposure to chemicals in air and soil, characterisation of health risks.
12. Estimation of exposure to chemical and biological agents in water, characterisation of health risks.
13. Estimation of exposure to chemicals from products and materials, characterisation of health risks.
14. Estimates of exposure to chemical and biological agents from food and food, characterisation of health risks.
15. Estimate of exposure by biomarkers.
16. Estimation of exposure and health risks from the working environment.
17. Selected statistical methods, data modelling, exposure models used in health risk assessment.

1. Identification of the hazards of agents - essential dangerous properties and adverse effects of physical, chemical and biological agents.
2. Assessment of dose-response relationship - determination of the relationship between exposure and expression or intensity of effect.
3. Exposure estimate - assessment of the sources of hazardous agents, exposure pathways and sites of interaction with the organism, determination of exposure or dose values, fate of chemicals in the organism, their transformation and excretion; a description of the exposed population.
4. Health risk characterisation - an estimate of the occurrence and intensity of adverse effects of agents that may occur in the population of individuals due to actual or anticipated exposure and which includes a quantitative estimate or assessment of the health risk.
5. Assessment of impact on public health, negative and positive effects, impact of socio-economic factors, perception of health risks and communication in the field of health risks.
