DECLARATION
of 31 October 2006
amending Decree No. 223 / 2005 Coll., on certain educational documents
The Ministry of Education, Youth and Sports provides, pursuant to § 28 paragraphs 7 and 8 of Act No. 561 / 2004 Coll., on pre-school, primary, secondary, higher vocational and other education (Education Act), as amended by Act No. 179 / 2006 Coll.:
Decree No 223 / 2005 Coll., on certain educational documents, is amended as follows:
1. Paragraph 1 (7), including footnote 2, reads:
"(7) The location of the school's department identifier (2) shall be indicated on the relevant forms by the abbreviation" IZO. "
2) Paragraph 144 (1) (a) of the Education Act. '
2. Paragraph 3 (6), including footnotes 5a and 5b, reads as follows:
"(6) Where a copy is issued due to a change in the sex of the applicant, the name and surname of the applicant and his / her birth number shall be indicated on the copy as at the date of issue of the copy. In this case, the application for the issue of a copy shall be accompanied by a decision by the competent administrative authority authorising the amendment of the name and surname (5a) and, where appropriate, an officially certified copy thereof, and an original of the certificate, a certificate of discharge or a diploma of discharge. Furthermore, the applicant shall, in accordance with the specific legislation5b), document the birth number assigned to him.
5a) Paragraph 74 (1) of Act No. 301 / 2000 Coll., on Matrices, Name and Surname and on the amendment of certain related laws.
5b) Article 15 of Act No. 133 / 2000 Coll., on the registration of residents and birth numbers and on the amendment of certain laws (Act on the registration of residents), as amended by Act No. 53 / 2004 Coll., Act No. 161 / 2006 Coll. and Act No. 165 / 2006 Coll. '
3. In Annex 1, the model form "Certificate for primary schools special - p. 1 - 4 'reads as follows:

4. In Annex 1, the model form "Certificate for primary schools special - Polish - p. 1 - 4 'reads as follows:
"'.
5. On the first page of Annex No 3, the words "Annual certificate - 16 compulsory subjects - faces and rump 'shall be inserted after the words" Annual certificate - 18 compulsory subjects - faces and rump'.
6. In Annex 3, the following model form of the certificate shall be inserted after the model form "Annual Certificate - 16 compulsory subjects - cheeks and sleeves':

7. The model form for the certificate in Annex 9 reads:
"'.
This decree shall take effect on the day of its publication.
