DECLARATION
of 26 October 2006
on the formalities for the certification of staff of sickness insurance authorities responsible for checking compliance with the temporary work scheme of an incapacitated insured person
The Ministry of Labour and Social Affairs provides according to § 199 (1) of Act No. 187 / 2006 Coll., on sickness insurance:
The card of the staff member of the sickness insurance authority responsible for checking compliance with the temporary work scheme of the incompetent insured person shall include:
(a) the designation "The document of the staff member responsible for inspection,"
(b) the name of the sickness insurance authority which issued the licence;
(c) a large national character;
(d) a photograph of an employee meeting the requirements laid down for photographs for the issue of a citizen's card (1);
(e) the title, name and surname of the staff member, where appropriate;
(f) the personal number of the staff member;
(g) control authorisation expressed in words "The holder of this licence is authorised to carry out a check of compliance with the temporary work scheme of an incompetent insured person under Act No. 187 / 2006 Coll., on sickness insurance ',
(h) the licence number;
(i) the date of issue of the licence;
(j) the official stamp of the sickness insurance authority and the signature of the person authorised to issue the licence.
This decree takes effect on the date on which Act No. 187 / 2006 Coll., on sickness insurance takes effect.
