DECLARATION
of 12 October 2006
amending Decree No. 490 / 2000 of the Ministry of Health Coll., on the scope of knowledge and other conditions for obtaining professional competence in certain fields of public health protection
According to Article 108 (1) of the Act No. 258 / 2000 Coll., on the Protection of Public Health and on the Amendment of Certain Related Acts, as amended by Act No. 274 / 2001 Coll., Act No. 320 / 2002 Coll., Act No. 274 / 2003 Coll., Act No. 392 / 2005 Coll. and Act No. 222 / 2006 Coll., ("the Act ') for the implementation of § 19 (3), § 59 (2), § 60 (1) and (3) and § 83e (2) Act:
Čl. I
Decree No. 490 / 2000 Coll., on the extent of knowledge and other conditions for obtaining professional competence in certain fields of public health protection, is amended as follows:

„§ 1
Preliminary provisions
This decree regulates
(a) the content and scope of the courses in the field of special protective disinfection, disinsectisation and deratization, the extent of the knowledge necessary to protect public health in the performance of epidemiological activities and the knowledge to be demonstrated by a person in the proficiency test to carry out health risk assessments;
(b) the particulars of the application for a proficiency test in the field of special protective disinfection, disinsecting and exerting and the essential content of the test;
(c) the composition of the test committees and the conditions for carrying out the examination in the fields referred to in (a). ';
2. in Article 3 (1) (a), the words "place of residence," shall be replaced by the words "address of the place of permanent residence or, where appropriate, residence, if the person does not have permanent residence in the Czech Republic,";
3. in Article 3 (1), point (b) is deleted;
Points (c) and (d) shall be renumbered (b) and (c).
4. In Article 3 (2), the words "Article 58 (2) to (4) and (5) (c) 'are replaced by" Article 58 (1) to (3)' and the words "if a person has completed it 'are added at the end of the text.
5. In Article 7 (3), the word "district 'is replaced by" regional'.
6. footnote 1 shall read:
"1) § 82 of Act No. 258 / 2000 Coll., on the Protection of Public Health and on the Amendment of Certain Related Acts, as amended by Act No. 274 / 2003 Coll. and Act No. 392 / 2005 Coll. '.
7. Sections 8 and 9 are deleted.

„§ 11
Paragraphs 5 (2) and 7 (1) and (3) shall apply mutatis mutandis to the test and its conduct. ';
9. The following Section 11a is inserted after Section 11:
„§ 11a
Proficiency test to carry out health risk assessments
(1) The knowledge test necessary to carry out health risk assessments shall consist of:
(a) the written part in which the applicant is to demonstrate general knowledge of the principles of health risk assessment as set out in Part A of Annex 4 to this Regulation; and
(b) the oral part in which the applicant is to demonstrate general knowledge of the methods of assessing health risks and knowledge to the extent of the assessment of health risks of exposures in the specific areas referred to in Annex 4 to this Decree.
The applicant shall perform the written part of the examination before the oral examination. A condition for the acceptance of the oral part of the proficiency test to carry out the assessment of health risks shall be the successful performance of the written part, which shall consist in the correct answers to at least two thirds of the questions raised. The test is not public.
(2) The examination committee set up for the proficiency test to obtain authorisation for the assessment of health risks shall be of three members. It shall consist of the President and two other members. Only a university-educated person who has been active for at least 5 years in the health risk assessment section may be appointed as chairman of the Examination Board and its members.
(3) Paragraph 6 and 7 shall apply mutatis mutandis to the information of the applicant for the examination and the course of the examination. The test report shall be kept with the authorising officer for 10 years.
(4) The knowledge which the applicant must demonstrate during the proficiency test to obtain authorisation for carrying out health risk assessments is set out in Annex 4 to this Decree. '
10. In Annex 1, point 1, the words "paragraph 2 'are replaced by the words" paragraph 1'.
11. In Annex No 1, point 1, the section Contents of the heading the text of subpoint 4 shall read:
"4. Content of the legislation on protective disinfection, disinsectisation and extermination according to the law in the scope of the obligations of persons in the field of special protective disinfection, disinsecting and exerting 4 hours'.
12. In Annex No 1, point 2, "paragraph 3 'is replaced by" paragraph 2';
13. In Annex 1, point 3, the words "paragraph 4 'are replaced by the words" paragraph 3'.
14. Annex No 2 is deleted.
15. In Annex 3, point 3 is added:
'3. The knowledge required to protect public health in terms of the prevention of water-related diseases in persons coming into direct contact with water in the course of their working activities in water treatment plants and in the operation of water pipelines are:
(a) health requirements for persons carrying out the relevant activity;
(b) principles of personal hygiene at work;
(c) principles of hygienically sound operation and maintenance of water facilities;
(d) 1. basic knowledge of the causes, epidemiology and principles of prevention and spread of diseases in which water may be involved;
2. basic knowledge of the causes of water poisoning;
(e) special hygiene issues, depending on work activity, within the scope of the operating rules of the water treatment plant or water supply plant. ';
16. Annex 4 shall read as follows:

"Annex No 4 to Decree No 490 / 2000 Coll.
Knowledge required to test competence to obtain authorisation for the assessment of health risks
A. General knowledge of the principles of health risk assessment:
1. The basis of the health risk assessment system and its link to epidemiology.
2. Hazardous noise properties and exposure / biological activity relationship.
3. Dangerous properties of non-ionising radiation and the relationship of exposure and biological effect.
4. Dangerous properties of chemicals and dust.
5. Dangerous properties of biological agents and relationship of exposure and biological effect.
6. Identification of hazards and selected chapters from toxicology, i.e. reproductive and developmental toxicity, neurotoxicity, genotoxicity, carcinogenicity, allergic hypersensitivity, hepatotoxicity, nephrotoxicity, immunotoxicity, local and systemic effects, acute and chronic effects of substances.
7. Dose (exposure) relationship and chemical response.
8. Estimation of exposure to biological agents in the environment, characterisation of health risks.
9. Estimation of exposure to noise in the environment and internal environment, characterisation of health risks.
10. Estimates of exposure to non-ionising radiation in the environment and the internal environment, characterisation of health risks.
11. Estimation of exposure to chemicals in air and soil, characterisation of health risks.
12. Estimation of exposure to chemical and biological agents in water, characterisation of health risks.
13. Estimation of exposure to chemicals from products and materials, characterisation of health risks.
14. Estimates of exposure to chemical and biological agents from food and food, characterisation of health risks.
15. Estimate of exposure by biomarkers.
16. Estimation of exposure and health risks from the working environment.
17. Selected statistical methods, data modelling, exposure models used in health risk assessment.
18. Relationship of laboratory data to exposure assessment.
19. Principles for describing and characterising health risks.
B. General knowledge of methods for assessing health risks.
1. Identification of the hazards of agents - essential dangerous properties and adverse effects of physical, chemical and biological agents.
2. Assessment of dose-response relationship - determination of the relationship between exposure and expression or intensity of effect.
3. Exposure estimate - assessment of the sources of hazardous agents, exposure pathways and sites of interaction with the organism, determination of exposure or dose values, fate of chemicals in the organism, their transformation and excretion; a description of the exposed population.
4. Health risk characterisation - an estimate of the occurrence and intensity of adverse effects of agents that may occur in the population of individuals due to actual or anticipated exposure and which includes a quantitative estimate or assessment of the health risk.
5. Assessment of impact on public health, negative and positive effects, impact of socio-economic factors, perception of health risks and communication in the field of health risks. "
Čl. II
Transitional provision
Certificates issued before the date of entry into force of this Order shall be deemed to have been issued under this Order.
Čl. III
Efficacy
This Decree shall take effect on 1 November 2006, with the exception of point 15, which shall take effect on 1 February 2007.
