DECLARATION
of 2 February 2012
on models of service card staff in the Financial Administration of the Czech Republic
The Ministry of Finance provides according to § 16 paragraph 2 of Act No. 456 / 2011 Coll., on the Financial Administration of the Czech Republic:
The model of the service pass shall be as follows:
(a) a model service card of polycarbonate; and
(b) a specimen of the service licence printed on the price paper.
The models of the service pass are set out in Annexes 1 and 2 to this Decree.
The service card issued in accordance with the model of the service card of polycarbonate and the service card issued in accordance with the model of the service card printed on the price paper shall be interchangeable when proving the staff member's entitlement to exercise the authority of the Financial Administration of the Czech Republic.
This Decree shall take effect on 1 January 2013.

Model of service cards of polycarbonate
1. Description
a) The service card is a two-sided white-coloured polycarbonate card which complies with ISO / IEC 7810: 1966 ID - 1, size 85,60 x 53,98 mm. The service card shall be provided by special security features. One of the special locking features is a holographic element with a large national character motif.
(b) The heading of the FINANCIAL GOVERNMENT OF THE CZECH REPUBLIC is shown on the front page of the service card. The second line shows the heading OF THE FINANCIAL GOVERNANCE AUTHORITIES OF THE CZECH REPUBLIC. Below it is a place for the name of the authority of the Financial Administration of the Czech Republic whose powers are directly involved. The third line contains the heading OF THE STAFF'S PERSONAL NUMBER and below it is the place for the employee's personal number. The fourth line contains the heading RECEIVATION and next to it is the place for the employee's surname. On the fifth line is the heading JMENO (A) and next to it is the place for the name or the names of the employee. On the sixth line is the title Titul and next to it is the place for the title of employee. On the seventh line, the heading of the DATE OF ISSUING THE SERVICE CERTIFICATE appears and below is the place for the date of issue of the service card. In the left part there is a colour representation of the employee, which meets the technical requirements of the ID photos. In the right part of the top is a representation of a large state emblem in a blue and white design and under it is a reserved place for electronically printed signature of the Director General of the General Financial Directorate. In the right part of the mine is the text "THE GENERAL FINANCIAL DIRECTORATE '. The service card number is given at the bottom of the centre.
c) On the reverse side of the service card, the following text is added: "The holder of this service card is an employee included in the authority of the Financial Administration of the Czech Republic and is entitled to exercise the authority of the Financial Administration of the Czech Republic." The text below is given: "The finder of this service card is requested to surrender it to the nearest authority of the Financial Administration of the Czech Republic." The contact chip is located on the left. Part of the service card is a contactless chip that is not visible.

Model of service licence printed on price paper
1. Description
(a) The service licence shall be 99 x 69 mm and shall be stored in a plastic package of 105 x 75 mm. The security features are the price paper with watermark, the price grid with a combination of colours that makes it difficult to obtain copies on colour laser printers and numbering with a serial number in colour that is displayed with red fluorescence in the ultraviolet spectrum. The requirements for the technical execution of the photograph shall comply with the technical requirements for photographs for the issue of the identity card.
b) The heading of the Financial Administration of the Czech Republic is shown on the front page of the service card. The second line shows the name of the Financial Administration of the Czech Republic, whose powers the employee is directly involved in. The third line shows the personal number of the employee, the fourth line shows his title, name and surname, if any. The fifth line indicates the Directorate-General for Finance as the authority issuing the service card. Under the fifth line is a dedicated place for electronically printed signature of the Director-General of the General Financial Directorate. The sixth line shall show the date of issue of the service card. There's a photo of the employee in the right side. A series and service card number are given below. There's a big state emblem on the left side of the top.
c) On the reverse side of the service card, the following text is added: "The holder of this service card is an employee included in the authority of the Financial Administration of the Czech Republic and is entitled to exercise the authority of the Financial Administration of the Czech Republic." The text below is given: "The finder of this service card is requested to surrender it to the nearest authority of the Financial Administration of the Czech Republic."
