DECLARATION
of 13 December 2024
on the details of the formalities for and the procedure for issuing the replacement travel document of the European Union
The Ministry of Foreign Affairs provides, pursuant to § 31b (7) and § 31d (3) of Act No. 150 / 2017 Coll., on Foreign Service and on the amendment of certain laws (Act on Foreign Service), as amended by Act No. 453 / 2024 Coll. ("the Act '):
Preliminary provisions
This decree implements the relevant provisions of the European Union1) and sets out the models of the single form of the European Union replacement travel document, the single label of the European Union replacement travel document and the application for the issue of the European Union replacement travel document, the procedure for the issue of the European Union replacement travel document and the scope of the machine-readable data entered in the machine-readable zone.
Single form of the European Union replacement travel document
(1) The model of the uniform form for the replacement travel document of the European Union is set out in Annex 1 thereto.
(2) The uniform form of the replacement travel document of the European Union shall have the design of a double-printed sheet folded into thirds, the size of which shall be 125 x 88 millimetres.
(3) The title page of the single form of the European Union replacement travel document contains the words "EUROPEAN UNION" in all the official languages of the European Union, followed by the words "EMERGENCY TRAVEL DOCUMENT" and "TITRE DE VOYAGE PROVISOIRE." The front page also shows twelve gold stars in a circle.
(4) The other side of the single form of the European Union replacement travel document shall be reserved for the affixing of the single label of the European Union replacement travel document.
(5) On the third and fourth pages of the single form of the European Union replacement travel document, the words "replacement travel document 'and the register of the names of the relevant data entered in the European Union replacement travel document shall appear in all the official languages of the European Union except in English and French, and the following text shall be added:
"This EU Emergency Travel Document is a travel document issued by a Member State of the European Union for a single journal to the holder's Member State of nationality or residence or, exceptionally, to another destination. Authorities of non-EU countries are hereby requested to allow the holder to pass freely without Hindrance.
Le présent titre de voyage provisoire de l'UE est un titre de voyage délivré par un État membre de l'Union européenne aux fins d'un trajet unique vers l'État membre de nationalité ou de résidence du détenteur, ou, à titre exctionnel, vers une autre destination. Les autorités des pays tiers sont priées d'autoriser le détenteur du titre de voyage provisoire à circuler sans traves. '
(6) The fifth and sixth pages of the single form of the replacement travel document of the European Union are entitled "VISA / VISA" and remain blank when the replacement travel document of the European Union is transmitted. These parties are reserved for visas and entry and exit stamps.
(7) A unique seven-digit number shall be pre-printed on the European Union replacement travel document form.
Single European Union replacement travel document label
(1) The model of the single label for the replacement travel document of the European Union is set out in Annex 2 thereto.
(2) The single European Union replacement travel document label shall contain:
(a) information on the non-represented citizen pursuant to Article 31d (2) (a) of the Act;
(b) information on the replacement travel document of the European Union pursuant to Article 31d (2) (b) of the Act
1. the number shall be the unique seven-digit number of the single label of the European Union replacement travel document pre-printed in black horizontally in its upper right corner and both lower corners;
2. document code means the three-letter code "EUE,"
3. the code of the Czech Republic means the two-letter code CZ given before the number of the single label of the European Union replacement travel document in the upper right-hand corner,
(c) the indication of the State to which the non-represented citizen is requested to travel or, where appropriate, the transit countries to which the non-represented citizen is to travel;
(d) the number of the European Union replacement travel document form to which it is attached;
(e) abbreviation of the document name in the form of "EU ETD / TVP-UE" and the words "European Union / Union européenne,"
(f) the section "Notes" to indicate other necessary and not duplicate national alerts.
(3) The indication of the relevant data entered on the single label of the European Union replacement travel document shall be given in English and French and shall be numbered ascending.
(4) In addition, the single European Union replacement travel document label shall contain machine-readable data, which shall be recorded in the manner specified in International Civil Aviation Organization document No 9303 on machine-readable travel documents (seventh edition, 2015) and shall indicate in order the type of document marked in capital letters "PU ', the code of the issuing State, the surname, the name or, where applicable, the name of the non-represented citizen, the number of the single label of the replacement travel document of the European Union, the number of the uniform form of the replacement travel document of the European Union, the nationality, the date of birth and the sex of the non-represented citizen, the expiry date of the single label of the replacement travel document of the European Union and the control numbers which are the numerical expression of the selected data in the machine-readable zone. The machine-readable data are shown at the bottom of the label. The words' European Union 'in all the official languages of the European Union are included in the print.
Requirements for the acquisition of digitised form and the execution of a photograph of an unrepresented citizen for the issue of a European Union replacement travel document
The digitised form of the photograph or photograph attached pursuant to Section 31b (5) of the Act must comply with the requirements of Part 3 of the International Civil Aviation Organisation (International Civil Aviation Organisation) No 9303 on machine-readable travel documents (seventh edition, 2015).
Technical conditions for drawing up the European Union travel document
(1) The single European Union replacement travel document label shall be affixed to the other side of the single European Union replacement travel document form so that the machine-readable zone is aligned to the outer edge of the Party. The stamp of the issuing representative office shall be printed on the bottom of the single label of the European Union replacement travel document in order to interfere with the uniform form of the European Union replacement travel document in the part under the label.
(2) The data on the single label of the replacement travel document of the European Union shall be given in the format "DD.MM. YYYY '.
(3) The information on the single label of the European Union replacement travel document referred to in Article 3 (2) to (4) must be printed. In the event of technical difficulties caused by force majeure, an exceptionally uniform European Union replacement travel document label may be completed by hand, attached to the photograph and provided with additional protection before replacement. No changes shall be made to the handwritten single label of the European Union replacement travel document.
(4) Where an error is detected on the single label of the European Union replacement travel document, the single label of the European Union replacement travel document concerned shall be invalidated and destroyed; If it has already been attached to the single form of the European Union replacement travel document, it shall be invalidated and destroyed together with the uniform form of the European Union replacement travel document.
Form of application for the European Union replacement travel document
The model of the application form for the issue of the European Union replacement travel document is set out in Annex 3 to this Order.
Efficacy
This Decree shall take effect on 9 December 2025.
