DECLARATION
Czech Mining Office
of 16 October 2002
on the reporting of serious events and of hazardous conditions, serious accidents (accidents), serious accidents at work and technical equipment disturbances
Pursuant to Article 6 (6) (d) of Act No. 61 / 1988 Coll., on Mining Activities, Explosives and the State Mining Administration, as amended by Acts No. 425 / 1990 Coll., No. 542 / 1991 Coll., No. 169 / 1993 Coll., No. 128 / 1999 Coll., No. 71 / 2000 Coll., No. 124 / 2000 Coll., No. 315 / 2001 Coll. and No. 206 / 2002 Coll., ("the Act '):
The report referred to in Article 6 (3) (b) of the Act shall state:
(a) serious accidents at work
1)
2. place, day, hour and registration number of the accident, 2)
3. the name and surname of the disabled (s),
4. type of injury and injury to the body,
5. species and source of injury,
(b) in the case of other major events, hazardous situations, major accidents (accidents) or technical equipment disturbances ("emergency"),

2. place, day and hour of emergency,
3. type of incident;
4. probable cause of an emergency,
5. estimated extent of damage.
The report referred to in Article 1 shall be without prejudice to the obligation of the organisation1) to notify to the Regional Mining Authority accidents at work and to send an accident alert under a special legislation.2)
Efficacy
This Decree shall take effect on 1 November 2002.
