DECLARATION
of 31 August 2006
on the formalities for the certification stamp of the holder of the postal licence and the certification stamp of the Chamber of Commerce of the Czech Republic (Order on the formalities for the verification stamp)
The Ministry of Informatics provides pursuant to § 12 paragraph 1 (j) of Act No. 365 / 2000 Coll., on Information Systems of Public Administration and on the amendment of certain other laws, as amended by Act No. 81 / 2006 Coll.:
Subject matter
This decree sets out the formalities for the certification stamp of the holder of the postal licence and the certification stamp of the Chamber of Commerce of the Czech Republic.
Forms of the certification stamp of the holder of the postal licence
(1) The stamp of the holder of the postal licence shall contain the following particulars:
(a) the company or the name of the holder of the postal licence, if he is a legal person, the name and surname and, where appropriate, the distinguishing supplement or further indication, if he is a natural person;
(b) the pictorial symbol of the postal licence holder, if used by the postal licence holder;
(c) the name of the establishment of the holder of the postal licence (post office);
(d) postal code of the post office,
(e) the serial number of the stamp distinguishing between the individual verification stamps allocated to one post office.
(2) The verification stamp of the holder of the postal licence shall be provided in such a way that his fingerprint shows the date at the time of verification of the exit from the public administration information system.
(3) The certification stamp of the holder of the postal licence shall not be interchangeable with the stamps of which the holder of the postal licence bears the surface of the postal consignment.
(4) The model of the certification stamp of the holder of the postal licence is given in point 1 of the Annex to this Order.
Forms of the verification stamp of the Chamber of Commerce of the Czech Republic
(1) The verification stamp of the Chamber of Commerce of the Czech Republic contains the following particulars:
(a) the name "Chamber of Commerce of the Czech Republic,"
(b) the pictorial symbol of the Chamber of Commerce of the Czech Republic,
(c) the name of the chamber office;
(d) the code name clearly identified by the office of the chamber;
(e) the serial number of the stamp distinguishing the individual verification stamps assigned to one office of the Chamber.
(2) The verification stamp of the Chamber of Commerce of the Czech Republic must be so equipped that its imprint shows the date at the time of verification of the exit from the public administration information system.
(3) The model of the verification stamp of the Chamber of Commerce of the Czech Republic is set out in point 2 of the Annex to this Decree.
Efficacy
This Decree shall take effect on 1 January 2007.

1. Model of the verification stamp of the holder of the postal licence
Stamp height is 36 mm. Stamp width is 36 mm.
On the perimeter of the stamp:
(a) the name of the post office;
(b) the postal code.
In the middle of the stamp:
(a) the commercial firm or the name of the holder of the postal licence, if he is a legal person, or the name and surname and, where appropriate, the distinguishing supplement or further indication if he is a natural person;
(b) the image symbol (logo) of the postal licence holder, if used by the postal licence holder;
(c) the current date,
(d) the serial number of the stamp.
2. Model verification stamp of the Czech Chamber of Commerce
Stamp height is 36 mm. Stamp width is 36 mm.
On the perimeter of the stamp:
(a) the name of the chamber office;
(b) a numerical indication clearly identified by the office of the chamber.
In the middle of the stamp:
(a) the name "Chamber of Commerce of the Czech Republic,"
b) image symbol (logo) of the Economic Chamber of the Czech Republic,
(c) the current date,
(d) the serial number of the stamp.
