DECLARATION
of 17 December 2024
amending Decree No. 3 / 2015 Coll., on certain documents relating to education, as amended
The Ministry of Education, Youth and Sports provides pursuant to § 28 (7) of Act No. 561 / 2004 Coll., on Pre-School, Basic, Medium, Higher Vocational and Other Education (Education Act), as amended by Act No. 179 / 2006 Coll. and Act No. 82 / 2015 Coll.:
Decree No. 3 / 2015 Coll., on certain educational documents, as amended by Decree No. 58 / 2016 Coll., Decree No. 300 / 2018 Coll., Decree No. 405 / 2020 Coll., Decree No. 200 / 2021 Coll. and Decree No. 530 / 2021 Coll., are amended as follows:
1. The following Section 2a is inserted after Section 2, including the title:
Models of forms for primary school evaluation verbally
(1) The primary school of assessment shall use the model forms set out in Annex 1 or Annex 1a for the purpose of assessing the results of the education of the pupil at the end of the semester.
(2) The primary school shall use the forms set out in Annex 3 or Annex 3a for the evaluation of the results of the training in the event of the achievement of the degree of education. ';
2. The following Annex 1a is inserted after Annex 1, including the title:

Certificate for primary school
1a.1 Certificate for primary school (verbal evaluation)
1a.2 Certificate for primary school (verbal evaluation) - Polish

3. The following Annex 3a is inserted after Annex 3, including the title:

Efficacy
This Decree shall take effect on 1 January 2025.
