Decree No. 430 / 2012 Coll.

Annex No 1 to Decree No. 47 / 2012 Coll., on the models of the staff member's service card in the Financial Administration of the Czech Republic, reads as follows:

1. Description

a) The service card is a two-sided white-coloured polycarbonate card which complies with ISO / IEC 7810: 1966 ID - 1, size 85,60 x 53,98 mm. The service card shall be provided by special security features. One of the special locking features is a holographic element with a large national character motif.

(b) The heading of the FINANCIAL GOVERNMENT OF THE CZECH REPUBLIC is shown on the front page of the service card. The second line shows the heading OF THE FINANCIAL GOVERNANCE AUTHORITIES OF THE CZECH REPUBLIC. Below it is a place for the name of the authority of the Financial Administration of the Czech Republic whose powers are directly involved. The third line contains the heading OF THE STAFF'S PERSONAL NUMBER and below it is the place for the employee's personal number. The fourth line contains the heading RECEIVATION and next to it is the place for the employee's surname. On the fifth line is the heading JMENO (A) and next to it is the place for the name or the names of the employee. On the sixth line is the title Titul and next to it is the place for the title of employee. On the seventh line, the heading of the DATE OF ISSUING THE SERVICE CERTIFICATE appears and below is the place for the date of issue of the service card. In the left part there is a colour representation of the employee, which meets the technical requirements of the ID photos. In the right part of the top is a representation of a large state emblem in a blue and white design and under it is a reserved place for electronically printed signature of the Director General of the General Financial Directorate. In the right part of the mine is the text "THE GENERAL FINANCIAL DIRECTORATE '. The service card number is given at the bottom of the centre.

c) On the reverse side of the service card, the following text is added: "The holder of this service card is an employee included in the authority of the Financial Administration of the Czech Republic and is entitled to exercise the authority of the Financial Administration of the Czech Republic." The text below is given: "The finder of this service card is requested to surrender it to the nearest authority of the Financial Administration of the Czech Republic." The contact chip is located on the left. Part of the service card is a contactless chip that is not visible.

This Decree shall take effect on 1 January 2013.
