DECLARATION
of 13 December 2013
on model documents under the Act on Citizenship of the Czech Republic
The Ministry of Interior provides pursuant to § 79 of Act No. 186 / 2013 Coll., on Citizenship of the Czech Republic and amending certain laws (Act on Citizenship of the Czech Republic) ("the Act '):
Charter on the acquisition of citizenship of the Czech Republic by grant
The model of the Act on the acquisition of citizenship of the Czech Republic is set out in Annex 1 to this decree.
Charter on Acquisition of Citizenship of the Czech Republic by Declaration
(1) The model of the Act on the acquisition of citizenship of the Czech Republic by a declaration pursuant to § 31 to 33, 35 or 36 of the Act is set out in Annex 2 to this Decree.
(2) The model of the Act on the acquisition of citizenship of the Czech Republic is set out in Annex 3 to this Decree by a declaration pursuant to Article 34 of the Act.
Evidence of the disappearance of Czech citizenship
(1) The model of the document referred to in § 40 (7) (a) of the Act on the Extradition of Citizenship of the Czech Republic is set out in Annex 4 to this Decree.
(2) The model of the document provided for in Paragraph 40 (7) (b) of the Act on the Extradition of the Citizenship of the Czech Republic is set out in Annex 5 to this Decree.
(3) The model of the document provided for in Section 40 (9) of the Act on the Extradition of Citizenship of the Czech Republic is set out in Annex 6 to this Decree.
Certification
(1) The model of the certificate provided for in Article 43 of the Act is set out in Annex 7 to this Decree.
(2) A model certificate under Paragraph 43 of the Act, in which, upon request, information is given on how, and under which legislation, a natural person has acquired or lost Czech or Czechoslovak citizenship, or also that, at a certain date or time, he was a Czech national citizen or a Czechoslovak national citizen, or that the natural person is a national citizen of the Czech Republic from a certain date continuously until the date of issue of the certificate, is set out in Annex 8 to this Decree.
(3) A model certificate according to Section 48 of the Act on the fact that a natural person is not a national citizen of the Czech Republic at the date of its issue is set out in Annex 9 to this Decree.
Efficacy
This Decree shall take effect on 1 January 2014.
