DECLARATION
of 14 December 2011
on the model, formalities and execution of the social system card, the model, the particulars and the execution of the certificate of loss, theft, damage or destruction of the social systems card and the model of the Standardised Social Officer Record
The Ministry of Labour and Social Affairs provides pursuant to § 4a paragraph 1 and § 4b paragraph 7 of Act No. 73 / 2011 Coll., on the Labour Office of the Czech Republic and on the amendment of related laws, as amended by Act No. 366 / 2011 Coll.:
(1) The Social System Card (hereinafter referred to as the "Card") is a plastic card with dimensions of 54 x 86 mm in ID- 11 format.
(a) with an electronic data carrier, by a contact electronic chip and a magnetic stripe; or
(b) without an electronic data medium which has only the function of a disabled person's licence (TP, ZTP or ZTP / P).
(2) Instead of a plastic card, the card referred to in paragraph 1 (b) shall be issued as a paper card for the period up to 31 December 2013, which shall be sealed in a transparent laminating sheet.
(1) The following information is given on the front of the card:
(a) the name and, where applicable, the name and surname of the card holder;
(b) the period of validity of the card.
(2) In addition to the information provided for in paragraph 1, a card with the function of a disabled person's card shall also indicate the type of card of the disabled person (TP, ZTP or ZTP / P), accompanied by a symbol of the name of the person with complete or practical deafness or deaf-blind person, or persons completely or practically blind. The card has 23 mm x 17,8 mm image of the card holder on the front. The card referred to in Article 1 (2) shall also be stamped and signed by the authorised official on the front.
(3) On the back of the card is marked "Social System Card," logo and name "Labour Office of the Czech Republic," card number in the Single Information System of Labour and Social Affairs and, if the card is marked on the front of the card, the date of birth of the card holder is indicated. The card shall bear the signature of the card holder or his legal representative on the back.
(4) The card in execution referred to in Article 1 (1) (a) is also provided with the details required to fulfil the card's payment function, including the security features.
(5) The card in the electronic part contains:
(a) a payment application in accordance with the international standard EMV2),
(b) an identification application containing:
1. card number in the Single Information System of Labour and Social Affairs,
2. Authentication function and data for electronic card services in public administration information systems and other designated systems using card functions according to standard X.5093); and
3. the name and, where applicable, the name, surname and birth number of the card holder.
(1) Confirmation of the loss, theft, damage or destruction of the social system card (hereinafter referred to as the "certificate") is a form in A6 format with a logo and the name "Labour Office of the Czech Republic."
(2) The confirmation shall contain the following information:
(a) the name and / or surname of the holder of the replaced card;
(b) the number of the card replaced;
(c) the identity card or other identity card of the holder of the certificate at the same time as the certificate is presented, allowing identification of the holder of the card of a person with disabilities;
(d) the date of issue of the certificate and its expiry date;
(e) the identification of the authority which issued the certificate, the stamp and the signature of the authorised official;
(f) the signature of the holder of the certificate or, where appropriate, his legal representative.
(3) Where the replaced card was also used as a card for a person with disabilities, the certificate shall include, in addition to the information provided for in paragraph 2, the identification of the type of licence of a person with disabilities (TP, ZTP or ZTP / P), accompanied by a symbol of the designation of a person with complete or practical deafness or deaf-blind persons, or persons completely or practically blind.
The model of the social system card in the version referred to in § 1 (1) (a) without the status of a disabled person, the model of the social system card in the version referred to in § 1 (1) (a) with the function of the disabled person and the model of the social system card in the version referred to in § 1 (1) (b) are set out in Annex 1 to this decree. The model form of the certificate of loss, theft, damage or destruction of the social system card without the licence of a disabled person and the model form of the certificate of loss, theft, damage or destruction of the social systems card with the licence of a disabled person are set out in Annex 2 to this decree. The symbol for the designation of a person with complete or practical deafness, the symbol for the designation of a person with a deafness and the symbol for the designation of a person completely or practically blind are given in Annex 3 to this decree.
(1) The card referred to in Article 1 (1) (a) shall be issued when entitlement to the benefit is granted if the person who received the benefit is no longer the holder of the benefit. The card referred to in Article 1 (1) (b) and (2) shall be issued when a disabled person is granted a card.
(2) In the event that the card expires and continues to be entitled to the benefit which is paid through the card, or the card holder is also the holder of the card of a person with disabilities, the regional branch of the Czech Labour Office will issue a new card to the holder of the current card.
The model of the Standardised Record of a Social Worker is set out in Annex 4 to this Order.
This Decree shall take effect on 1 January 2012.
