DECLARATION

of 18 September 2002
on personal radiation cards
The State Nuclear Safety Authority provides, pursuant to § 47 (7), for the implementation of § 3 (2) (j), § 18 (1) (r) and § 6 of Act No. 18 / 1997 Coll., on the Peaceful Use of Nuclear Energy and Ionising Radiation (Atomic Act) and amending and supplementing certain laws, as amended by Act No. 13 / 2002 Coll., hereinafter referred to as "the Act":
Personal Radiation Card
(1) The personnel of a person who has received an authorisation pursuant to Article 9 (1) of the Act (hereinafter referred to as "the holder of the authorisation ') and who are temporarily assigned to carry out work as Category A1 workers) in the controlled zone of another authorisation holder (hereinafter referred to as" the operator of the controlled zone') shall be provided with a personal radiation licence as from 1 January 2004. The same requirement of a personal radiation licence shall apply to holders of authorisations who, as category A workers, carry out such activities personally for other operators of the controlled zone. For the purposes of this decree, the natural persons listed in the sentences of the first and the second shall be regarded as external personnel.
(2) The holder of an authorisation shall apply in writing to or for his staff to carry out the activities referred to in paragraph 1 by the State Nuclear Safety Authority (hereinafter "the Authority '). In the application for the issue of a personal radiation licence, the authorisation holder shall enter the particulars referred to in points 1 to 4 and 6 to 9 of Part A of the Annex to the Ordinance and attach a photograph of 3,5 x 4,5 cm.
(3) The Personal Radiation Card consists of two parts, Part A and Part B. Part B is used for recording doses in the current year and is valid every 1 calendar year. Part A of the Personal Radiation Card shall be valid until the data are filled, but not more than 10 years. The validity of both parts of the personal radiation licence shall also cease to be the death of an external worker or, if the external worker is the holder of an authorisation at the same time, by the expulsion of an authorisation pursuant to Article 16 (6) (b), (c) and (d) of the Act.
(4) The personal radiation licence shall be marked with the state emblem and name of the Office in the upper left corner and shall contain:
(a) the registration number;
(b) the identification marks of the external worker referred to in points 1 to 9 of Part A of the Annex to the Decree, including a photograph of 3,5 x 4,5 cm;
(c) records of the annual effective doses of an external worker;
(d) the name and address of the holder of the authorisation, including his registration number, the authorisation number issued by the Office;
(e) the beginning and end of the employment of an external worker by individual authorisation holders;
(f) records of preventive medical examinations carried out and their results;
(g) records of training in radiation protection;
(h) the results of the personal monitoring of the external worker of the individual operators of the controlled zone, including the name of the operator of the controlled zone, his workplace and his registration number.
(5) The model of the Personal Radiation Card is set out in the Annex to this Decree.
Handling a personal radiation card
(1) The holder of the authorisation for his staff or for himself, if the activity referred to in Article 1 (1) is to be carried out in person,
(a) request from the Office, in good time before the beginning of each calendar year, but not later than the end of November of the preceding year, a new Part B of the personal radiation licence;
(b) send to the Office, by the end of February of the following year at the latest, Part B of the Personal Radiation Card, where all external worker exposures from the previous year, including the assessed and confirmed total personal annual allowance, will be recorded;
(c) notify the Office without delay of the loss of a personal radiation licence and at the same time request the Office to issue a new licence in which to complete all necessary data up to the time of the loss of the licence;
(d) send part A of the personal radiation licence to the Office after it has been filled and request, no later than one month before the estimated expiry date of the personal radiation licence, a new personal radiation licence from the Office;
(e) send the personal radiation card to the Office no later than one month after the termination of an external worker's employment or the termination of his activity, provided that he is an external worker as the holder of the authorisation;
(f) notify the Office of changes to the data referred to in points 1, 7 or 8 of Part A of the Personal Radiation Card and submit the Office's Personal Radiation Card to record such changes;
(g) records in the personal radiation licence part And benefits received over the previous four-year period so that the five-year total personal benefit of an external worker can always be monitored,
(h) records in Part B a personal radiation certificate, issued during a calendar year, all personal benefits received by the worker until the time of fitting a personal radiation certificate in order to evaluate the total annual personal dose of the external worker;
(i) record in the relevant Part B of the Personal Radiation Card the monthly summary of the external worker's personal doses including the results of the monitoring of all operators of the controlled zones where the external worker has performed the activities referred to in Article 1 (1) during that month; the evaluation procedure for these monthly summaries must be specified in the approved monitoring programme of the authorisation holder, 2)
(j) indicate in Part A of the Personal Radiation Card the total personal annual benefits of an external worker;
(k) state the results of the annual medical examination on the basis of the medical report received;
(l) record of the radiation protection training completed.
(3) The operator of the controlled zone in which an external worker operates pursuant to Article 1 (1) shall ensure, in accordance with an approved monitoring programme for its controlled zone, an evaluation of the personal dose received by an external worker in carrying out that activity in its controlled zone. The assessed dose shall be recorded on a continuous basis, but not more than one month intervals, in the relevant Part B of the external personnel's personal radiation certificate. If the operator of the controlled zone does not have all the results of the measurement available before the end of the activity of an external worker in its controlled zone, he shall send those results in writing immediately after evaluation of the dose to the authorisation holder who is an external worker or whose employee is an external worker.
Efficacy
This decree shall take effect on the day of its publication.
