DECLARATION
of 14 December 2011
amending Decree No. 207 / 2001 Coll., implementing Act No. 301 / 2000 Coll., on Matrices, Name and Surname and on the amendment of certain related acts, as amended
According to § 96 (a) and (k) of Act No. 301 / 2000 Coll., on Matrices, Name and Surname and on the amendment of certain related laws:
Decree No. 207 / 2001 Coll., implementing Act No. 301 / 2000 Coll., on Matrices, Name and Surname, and on the Amendment of Certain Related Laws, as amended by Decree No. 230 / 2001 Coll., Decree No. 499 / 2001 Coll., Decree No. 566 / 2002 Coll., Decree No. 660 / 2004 Coll., Decree No. 539 / 2005 Coll., Decree No. 300 / 2006 Coll., Decree No. 352 / 2006 Coll., Decree No. 389 / 2007 Coll., Decree No. 419 / 2008 Coll., Decree No. 445 / 2008 Coll., Decree No. 485 / 2009 Coll. and Decree No. 315 / 2010 Coll., are amended as follows:
1. in Article 1, points (b) and (c) are deleted;
Points (d) to (f) shall be renumbered (b) to (d).
2. In Annex No 1, in the column "List of municipalities, cities, urban areas, urban districts and military exits of the administrative district of the matrix office ', the word" cities' in the title of this column is deleted.
3. In Annex 1, in the part of the Moravian-Silesian Region, in the column "Community of extended application ', in point 2." Bohumín', in the column "Matrimonial Office ', point 2 is inserted." Lower Lutyna' and in the column "List of municipalities, urban areas, urban districts and military exits of the administrative district of the Matrimonial Office ', point 1 is inserted after point 1." Lower Lutyna'.
In the column "Matrix Office ', point 2 is renumbered as point 3.
4. In Annex No 1, in the part of the Moravian-Silesian Region, in the column "Community with extended scope ', point 18." Eagle', in the column "Matrix Office 'and in the column" List of municipalities, urban areas, urban districts and military exits of the Administration Office', point 1 is deleted.
In the column "Matrix Office ', points 2 to 4 are renumbered as points 1 to 3.
5. In Annex 2 to Model 13, in the first paragraph, the words "Municipal office at, Das Gemeindeamt in, Myниципалитет в 'are deleted.
6. In Annex No 2 to Model 13, the words "the deceased partnership 'shall be inserted after the words" the widow', the words "the deceased partnership 'shall be inserted after the words" the terminated partnerships', the words "the terminated partnerships' shall be inserted after the words" Witwer / Witwe 'shall be inserted after the words "the erloschene Partnerschaft' and the words" вдовец-вдова 'shall be inserted after the words "the terminated partnerships', the words" the partnership 'shall be inserted after the words', the words "the EFTA / Witwer / Witwe 'shall be inserted, the words" the EFTA or the EFTA or the EFTA Republic of the EFTA Surveillance Authority'.
7. In Annex No 2 to Model 14, in the first paragraph, the words "Municipal office at, Das Gemeindeamt in, Myниципалитет в 'are deleted.
Matrix forms issued under existing legislation may be used from the date of entry into force of this Decree until 31 December 2020 at the latest, provided that the amendments referred to in points 5, 6 and 7 of Article I are indicated.
This Decree shall take effect on 1 January 2012.
