DECLARATION

amending Decree No 537 / 2006 Coll., on vaccination against infectious diseases, as amended
The Ministry of Health provides pursuant to § 108 (1) of Act No. 258 / 2000 Coll., on the Protection of Public Health and on the Amendment of Certain Related Acts, as amended by Act No. 274 / 2001 Coll., Act No. 320 / 2002 Coll., Act No. 274 / 2003 Coll., Act No. 392 / 2005 Coll., Act No. 222 / 2006 Coll., Act No. 151 / 2011 Coll., Act No. 223 / 2013 Coll. and Act No. 267 / 2015 Coll., for the implementation of § 47a paragraph 5:
Decree No. 537 / 2006 Coll., on vaccination against infectious diseases, as amended by Decree No. 65 / 2009 Coll., Decree No. 443 / 2009 Coll. and Decree No. 299 / 2010 Coll., is amended as follows:
1. In Article 1 (b), the word "a 'is deleted; at the end of the text in point (c), the dot is replaced by" a' and the following point (d) is added:
"(d) a model international certificate of vaccination against yellow fever in Czech and English."
2. After Paragraph 13, the following Section 13a is inserted:
International yellow fever vaccination certificate
The model of the international certificate of vaccination against yellow fever in Czech and English is set out in Annex 4 to this Regulation. '
3. The following Annex 4 is inserted after Annex 3:
"Annex No 4 to Decree No 537 / 2006 Coll.
MODEL INTERNATIONAL VACCINATION CERTIFICATE AGAINST RURAL IMMISSION
MODEL INTERNATIONAL CERTIFICATE OF VACCINATION AGAINST YELLOW FEVER
Efficacy
This Decision shall enter into force on 1 April 2016.
