DECLARATION
of 22 October 2008
on the Statistical Survey Programme 2009
The Czech Statistical Office provides pursuant to § 27 (b) of Act No. 89 / 1995 Coll., on the State Statistical Service, as amended by Act No. 411 / 2000 Coll.:
The 2009 statistical survey programme is set out in Annexes 1 and 2 to this Decree.
(1) In addition to the data described in Annexes 1 and 2 for each statistical survey, the statistical survey also contains the reporting agent's identification data for the legal entity, the trading firm or the name and its registered office and, for the trading entity, the name, surname and, where applicable, the business name and place of business. For both types of reporting agents, the statistical survey shall also contain an identification number, if not allocated, the tax identification number, the predominant activity in the reference period, the name or names, if any, and the surname of the person who completed the report, its telephone and fax numbers and the e-mail address, if any, if any. In the event that another entity fills in the statement for the reporting agent, the name and / or names of the person who completed the report shall also be given and the surname of the person who completed it, its telephone, fax and e-mail address, if any, if any.
(2) The statistical surveys concerning health, social services, cultural, school or educational establishments shall also include the internal department code names of the internal unit and the business or name and identification number of the founder or founder.
This Decree shall take effect on 1 January 2009.
