on the setting of fees for the activities of state professional surveillance organisations in the conduct of safety oversight of dedicated technical equipment
In agreement with the Ministry of Finance, the Ministry of Labour and Social Affairs provides, pursuant to § 6a (3) of Act No. 174 / 1968 Coll., on State Professional Labour Safety Supervision, as amended by Act No. 124 / 2000 Coll.:
The fees for the activities of state professional surveillance organisations in the conduct of safety oversight of reserved technical installations are set out in the Annex to this Decree.
This decree shall take effect on the day of its publication.

Annex to Decree No 398 / 2001 Coll.
Fees for the activities of state professional surveillance organisations in the conduct of safety oversight of reserved technical equipment pursuant to Act No. 174 / 1968 Coll., on State Professional Safety Supervision, as amended by Act No. 124 / 2000 Coll., hereinafter referred to as "the Act"
1. For the issue of a professional and binding opinion on whether technical equipment safety requirements according to § 6a (1) (a) of the Act, CZK 2,000 and CZK 1,250 are met for the design, construction, manufacture, assembly, operation, operation, operation, servicing, repair, maintenance and revision of dedicated technical equipment, and for each starting hour when processing this opinion.
2. In specified cases, for carrying out inspections, management and evaluation of tests to certify that the reserved technical equipment and materials used for their manufacture meet the requirements of the regulations for ensuring the safety of technical equipment and, in specified cases, for confirming the successful test results according to § 6a (1) (b) of the Act for each starting hour of CZK 1,250.
3. In specified cases for checking the professional competence of organisations and business natural persons for the production, assembly, repair, revision and testing of reserved technical equipment and for filling the containers with gases pursuant to § 6a (1) (c) of the Act for each hour started CZK 1 250.
4. For the certification of professional competence of organisations and business individuals for the production, assembly, repair, revision and testing of reserved technical equipment and for filling of containers of gases according to § 6a (1) (c) of the Act 3 000 CZK.
5. For changing the competence of organisations and business natural persons for the production, assembly, repair, revision and testing of reserved technical equipment and for filling of containers with gases in order to extend the scope of the authorisation according to § 6a (1) (c) of Act 650 CZK.
6. For changing the data in the approval of the professional competence of organisations and business individuals for the production, assembly, repair, revision and testing of reserved technical equipment and the filling of containers of gases pursuant to § 6a (1) (c) of Act 130 CZK.
7. For checking the competence of individuals for testing, revisions, repairs and assembly for one type, class or group of reserved technical equipment according to § 6a (1) (d) of the Act 3 400 CZK.
8. For checking the professional competence of natural persons for testing, revisions, repairs and assembly for one type, class or group of reserved technical equipment according to § 6a (1) (d) of the Act after five years of validity from the date of issue of the certificate CZK 1,600.
9. For checking the competence of natural persons for testing, revisions, repairs and assembly of dedicated technical equipment for the other and each other other type, class or group of dedicated technical equipment
(a) in order to extend the scope of the certificate pursuant to § 6a (1) (d) of Act 750 CZK,
b) after five years of validity from the date of issue of the certificate pursuant to § 6a (1) (d) of Act 750 CZK.
10. For the issue of a certificate of professional competence of individuals for testing, revisions, repairs and assembly of reserved technical equipment according to § 6a (1) (d) of the Act 200 CZK.
11. For changing the data in the certificate of competence of individuals for testing, revisions, repairs, assembly or operation of reserved technical equipment according to § 6a (1) (d) of the Act 100 CZK.
12. For checking the professional competence of natural persons to operate dedicated pressure equipment - boilers (hereinafter referred to as "boilers') and issuing the certificate referred to in § 6a (1) (d) of the Act for the basic examination including the relevant test supplementary to the
a) 1st class boilers 4 550 CZK,
b) 2nd class boilers 3 900 CZK,
c) 3rd class boilers 2 750 CZK,
d) 4th class boilers 2,000 CZK,
(e) for an additional test for another class of boilers equal to the difference between the fees of each class of boilers referred to in points (a) to (d);
(f) for a supplementary test for fuel other than that shown on the certificate CZK 700.
13. For checking the competence of natural persons to operate boilers and issuing certificates pursuant to § 6a (1) (d) of the Act after a period of five years from the date of issue of the certificate for the basic test including the relevant test supplementary to the
a) 1st class boilers 1,800 CZK,
b) 2nd class boilers 1,500 CZK,
c) 3rd class boilers 1 100 CZK,
d) 4th class boilers 850 CZK.
14. For the issue of each page of the copy of the opinion, certificate or authorisation according to § 6a (1) (a) to (d) of the Act 100 CZK.
