DECLARATION
of 17 December 2010
amending Decree No. 134 / 1998 Coll., which publishes a list of health performance with points, as amended
According to Article 17 (5) of Act No. 48 / 1997 Coll., on Public Health Insurance, and amending and supplementing certain related laws, as amended by Act No. 245 / 2006 Coll. and Act No. 261 / 2007 Coll.:
Decree No. 134 / 1998 Coll., issuing a list of health performance with point values, as amended by Decree No. 55 / 2000 Coll., Decree No. 135 / 2000 Coll., Order No. 449 / 2000 Coll., Decree No. 101 / 2002 Coll., Decree No. 291 / 2002 Coll., Decree No. 493 / 2005 Coll., Decree No. 620 / 2006 Coll., Decree No. 331 / 2007 Coll., Decree No. 439 / 2008 Coll., Decree No. 244 / 2009 Coll. and Decree No. 472 / 2009 Coll., is amended as follows:
1. In the Annex, in Chapter 2, item 11.2. at the end of the text in point 5, the words "including prescription drugs or ZP 'are added.
2. In the Annex, in Chapter 3, item 5.2, at the end of the text in point 6, the words "or, where appropriate, the request for further clinical, laboratory or imaging tests, including prescription drugs or ZP 'are added.
3. In the Annex, in Chapter 3, entry 6.2 at the end of the text in point 9, the words "including, where appropriate, the prescription of medicines or ZP 'are added.
4. In the Annex to Chapter 4, entry 1 reads as follows:

shall be reported:
1. to verify changes in glycaemic control if decompensation is suspected,
2. the determination of postprandial glucose in persons who do not have the possibility of self-monitoring and if it cannot be investigated from venous blood while receiving a biochemical test,
3. in those who are not treated for diabetes mellitus, the nature of the difficulties is considered to be the decompensation of diabetes mellitus.
Performance may not be reported as screening for preventive and dispensary examinations and also if venous blood is collected for biochemical testing. Other authorised expertise: 002, 003, 101, 103 and 104.

For the reporting of power, the conditions laid down for the performance number 02230 shall apply mutatis mutandis. ';
5. The following entry 42.2 is added to Chapter 4 of the Annex:

6. In the Annex to Chapter 7, headings 1 and 2 read:
"1. Minute overhead rate allocated to power
For the performance of the author's expertise 001, 002, 101, 102, 103, 104, 105, 106, 107, 108 (except for the nephrological haemolemination method), 109, 201, 202, 204, 205, 206, 207, 208, 209, 210, 222, 301, 302, 303, 304, 305, 306, 309, 401, 402, 403, 404, 405, 406, 407, 710, 801, 802, 804, 805, 807, 808, 809, 812, 813, 814, 815, 816, 817, 818, 819, 822, 823, 901, 902, 903, 904, 910, 917, 925, 931, 999 is assigned a minute-rate of ambit.
The performance of the author's expertise 115, 117, 215, 312, 413, 697 and 810 is assigned a minute overhead rate of 4,17 points per minute of power time.
The performance of the author's expertise 108 (only nephrological haematology methods), 203, 501, 502, 503, 504, 505, 506, 507, 601, 602, 603, 604, 605, 606, 607, 701, 702, 704, 705, 706, 707, 708, 709 is assigned a minute overhead rate of 3,33 points per minute of power time.
The performance of the author's expertise 511, 512, 513, 514, 515, 516, 517, 611, 612, 613, 614, 615, 616, 711, 714, 715, 716 and 717 is assigned a minute overhead rate of 4,99 points per minute of power.
The performance of the author's expertise 521, 522, 523, 524, 525, 526, 527, 621, 622, 623, 624, 625, 626, 721, 724, 725, 726, 727 and 728 is assigned a minute overhead rate of 6,66 points per minute of power.
The performance of the author's expertise 531, 532, 533, 534, 535, 536, 537, 631, 632, 633, 634, 635, 636, 731, 734, 735, 736 and 737 shall be assigned a minimum overhead rate of 9,99 points per minute of power.
The performance of the original expertise 911, 913, 914, 916, 921, 927 is assigned a minute overhead rate of 2,02 points per minute of power time.
The performance of the author's expertise 014, 015 is assigned a minute overhead rate of 3,47 points per minute of power.
The one-minute overhead rate for one minute of power allocated by this decree to the performance of individual copyright experts shall be calculated on 1 July of each calendar year by adding to the present value the average annual inflation rate (measured by the index of consumer prices) published by the Czech Statistical Office for the past year. The minute overhead rates thus calculated shall be rounded to two decimal places and shall apply from 1 January to 31 December of the following calendar year.
In a contract between a health insurance undertaking and a healthcare establishment, a higher rate of one minute than that indicated in the decree may be agreed on the basis of economically justified costs, but up to a maximum of 300% of that value.
The contract between the health insurance company and the health care establishment may, on the basis of the economically justified costs, agree a lower minute overhead rate than indicated in the decree, but at least 30% of that value.
2. Directed by OD
The other part of the overhead resulting from the different costs of individual bed health care facilities is covered by public health insurance in the form of a direction assigned to the exercise of the treatment day.
2.1. Bed care facilities providing emergency bed care
In one or more of the sectors contracted, they contract the value of overheads allocated to the treatment date at points in the range of 27,82 to 90,40 points.
The director assigned to the treatment day shall be established uniformly for the whole medical facility.
2.2. Medical facilities providing other bed health care
It is a health care institution which does not show treatment days, except for income and discharge tests, no other medical performance (in particular nursing beds, professional medical institutes, long-term sick hospitals and hospital-type special beds). The value of the direction assigned to the treatment date is set at 166,88 points.
The value of the overhead assigned by this decree to the treatment date shall be calculated on 1 July of the calendar year by adding to the current value the average annual inflation rate (measured by the index of consumer prices) for the previous year published by the Czech Statistical Office. The minute overhead rates thus calculated shall be rounded to two decimal places and shall apply from 1 January to 31 December of the following calendar year.
In a contract between a health insurance company and a healthcare establishment, a higher value than that assigned to OD may be agreed on the basis of economically justified costs, but up to a maximum of 400%.
In a contract between a health insurance company and a healthcare establishment, a lower value of the overhead assigned to OD than that indicated in the decree may be agreed on the basis of economically justified costs. ';
7. In the Annex to Chapter 001 - General practical medicine, the following power No 01445 is inserted after power No 01441:

8. In the Annex to Chapter 002 - Practical medicine for children and adolescents, Performance No 02128 is deleted.
9. In the Annex to Chapter 107 - cardiology, the following power No 17247 is inserted after power No 17244:
"17247 DETAILED CONTROL OF PATIENT WITH CARDIOSTIMULATOR AND IMPLANTABILITY CARDIOVERTER - DEFIBRILATOR
Perform a status check and correct function of a pacemaker or defibrillator providing remote control of the system by sending measured parameters using a mobile phone without the physical presence of the patient in an ambulance specialist

12. In the Annex to Chapter 817 - Clinical cytology laboratory, the following power No 95201 is inserted after power No 95199:
"95201 EXAMINATION OF THE PRESENCE OF NUCLEAR ACIDS OF HIGH HPV RISK TYPE IN THE CERVICAL CURRENT FOR SCREENING THE CARCINATION OF THE HRS
Performance is indicated for the prevention of cervical cancer in women with contested cytological findings and as control of conjunctive or ablative performance performed for the treatment of cervical dysplasia or microinvasive cervical cancer with a minimum interval of 6 + months after surgery. The power is performed by the Hybrid Capture 2 method or by automatic DNA isolation and subsequent amplification and detection of H.H.H.V. DNA.

13. In the Annex to Chapter 911 - General Nurse Performance No. 06129:
"06129 NEEDS AND INSULIN INSULIN INSULIN EDUCATION
The performance will be paid after the previous indication of the treating physician, which may be a single or expressing specific frequency of visits, but not more than three times a day for a maximum period of 14 days.

Efficacy
This Decree shall take effect on 1 January 2011.
