DECLARATION

National Security Office

of 9 November 2000

amending National Security Office Decree No 245 / 1998 Coll., on the personality and model of forms used in the field of personal security

The National Security Office provides pursuant to § 22 (5), § 33 (9) and § 47 (2) of Act No. 148 / 1998 Coll., on the protection of classified information and on the amendment of certain laws, as amended by Act No. 363 / 2000 Coll.:

Čl. I

Decree No. 245 / 1998 Coll., on the personality and model of forms used in the field of personal security, is amended as follows:

1. in Paragraph 1 (4) (a), the words "I and" shall be deleted;

„§ 4

Security questionnaire

The safety questionnaire is set out in Annex 1, provided in written or electronic form. '

3. The following Section 4a is inserted after Section 4:

„§ 4a

The model of the declaration of personality is set out in Annex 2. ';

4. In Section 5, "2 'is replaced by" 3'.

5. Annex 1 shall read as follows:

"Annex No 1 to Decree No. 245 / 1998 Coll.

QUESTION FOR PHYSICAL PERSONS SAFETY

6. Annex 2 shall read as follows:

"Annex No 2 to Decree No. 245 / 1998 Coll.

DECLARATION ON PERSONAL ELEMENTS

This decree shall take effect on 22 November 2000.
